Assessment of Dietary Intake of Preschool Children in Busan : Assessment Based on Food Group Intake

- To assess diet quality by food group intake and to investigate the interrelationship of age, dietary diversity score(DDS), dietary variety score(DVS), dietary frequency score(DFS), food group intake and nutrient intake with food group intake, a dietary survey was conducted with 176 preschool children aged 1 to 6 in Busan using a 24-hr recall method. Food group intake was assessed by food number consumed and intake frequency by six food groups(grain, meat, vegetable, fruit, dairy, sweets group). The mean food numbers consumed and intake frequencies by six feed group were 3.1 and 4.0 in the grain group, 3.6 and 4.0 in the meat group, 3.5 and 4.1 in the vegetable group, 1.0 and 1.1 in the fruit group, 1.3 and 1.5 in the dairy group, 1.4 and 1.4 in the sweets group respectively. As age increased, the intake frequency of the grain group(p<0.05) increased but that of the dairy group(p<0.05) decreased significantly. The DVS and DFS didn't show significant correlations with intake frequency of the dairy group. The grain group intake had significant positive correlations with intakes of the meat, vegetable, and fruit groups. The vegetable group intake had signigicant positive correlations with intakes of the grain and meat groups. The dairy group intake had significant positive correlation with sweets group intake but negative correlations with intakes of the grain and vegetable groups. As the intake frequency of the meat group increased, the NAR(nutrient adequacy ratios) of all nutrients and NAR(mean adequacy ratio) increased significantly. NARs of provein iron, vitamin B1, niacin had the highest correlation with the meat group intake and those of protein, calcium, phosphorous, and vitamin B2 had the highest correlation with the dairy group intake. NARs of vitamin A and vitamin C had the highest correlation with intake of the vegetable and fruit groups respectively. - The purpose of this study is to determine the prevalence of obesity and to examine the eating habits of elementary school students in Iksan dity. The study was carried out on 292 students(male: 151, female : 141)in May of 1997. The average height and weight of the subjects were 141 cm and 37 kg in males and 142 cm and 36 kg in females, which were similar to the average of Chonbuk. A Weight/Length Index(WLI), Broca Index, Rohrer Index(RI), Body Mass Index(BMI) and Waist Hip Ratio(WHR) were used as an obesity index to classify degree of obesity. WLI, Broca Index and RI were proper for judging child obesity and strict judgments were conducted in the order of RI, WLI and Brcoa Index. The subjects were classified as 30.8% underweight, 43.5% normal, 8.2% overweight and 17.5% obese by WLI and there were significant differences between height(p<0.05) and weight(p<0.01) according to obesity degree among the four groups. 50.3% of the boys and 55.3% of the girls are regularly, and the main reason they didn't ate regularly, and the maim reason they didn't eat breakfast(78.4%) was due to lack of time. The frequency of snacks were 1-2 times a week(40.4%), fruits(40.4%) from market and supermarket(68.2%). According to the survey 84.6%, 77.45, 78.1%, 69.9%, 59.9% and 86.6% of subjects didn't eat meat, fish, soysauce, instant food, sugar and seaweed respectively. Seaweed, meat, and soysauce showed low preferences. 69.5% and 74.75 of the subjects drink milk and ate kimchi respectively. By analyzing eating habits according to the degree of obesity, the rate of skipping breakfast(p<0.01), and eating meat or fish(p<0.05) were high in the overweight and obese groups of girls. In the boys case, the rate of not eating food in sugar(p<0.05) was high in the obese group. Obese children need to correct their eating habit. This reasserts the importance of nutrition education in children though teachers, parents, and dietitions. A Study of Iron Status and Anemia in Female High School Students in Ulsan

- This study was designed to assess the iron nutritional status and anemia of high school students. 383 female subjects in Ulsan Metropolitan city were evaluated using a questionnaire, and a measurement of hematological indices. The average height and weight of the respondents were 161.24$\pm$4.90 cm and 53.12$\pm$6.37kg, repectively. The average BMI(body mass index) was 20.43$\pm$2.26 which was in the normal ramge. The average hemoglobin(Hb) concentration of the subjects was 13.14$\pm$0.97g/dl, and the average hematocrit(Hct) level was 40.84$\pm$17.40%. Transferrin saturation{TS(%)} was 20.86$\pm$10.32%, and the ferritin by Hct(<36%), 27.2%by TS(<14%),26.6% by ferritin(<12 ng/ml). As for clinical symptoms, the greatest number of respondents reported that they experienced ‘decreased ability to concectrate’. Mean daily intakes of iron were 14.89$\pm$4.48 mg and heme iron intakes were 5.04$\pm$2.13 mg, which was 29.6% of total iron intake. The total iron binding capacity(TIBC) was negatively correlated with Hb concentration(r= -0.222, p<0.01). Serum ferritin was positively correlated with Hb concentration(r= - 0.323, p<0.05) and negatively correlated with TIBC(r= -0.367, p<0.01). TS(%) was positively correlated with Hb concentration(r= 0.402, p<0.01) and positively correlated with serum ferritin(r=0.413, p<0.01). As for the correlation between blood biochemistry and clinical symptoms related to anemia, the Hb concentration was negatively correlated with ‘shortening of breath when going upstairs(p>0.05)’ and ‘cold hands and feet’ significantly(r= -0.109, p<0.05). The level of Mean corpuscular volume(MCV) was negatively correlated with ‘feel dizzy when standing up’,‘tired out easily’, and ‘decrease ability to concentrate’ significantly(p<0.05). In particular, the level of Fe was negatively correlated with ‘shortening of breath when going upstairs’ and ‘feeling blue’ significantly(p<0.01). These results suggest That the prevalence of iron deficiency of female high school students is very high, therefore guidelines for diet supports and nutrition education to improve their iron status should be provided.

Attitudes of Food and Nutrition Majors toward Hiring Older Workers

- The purpose of this study was to identify the attitudes of nutrition majors toward hiring older workers who are over fifty-five years of age. Questionnaires were sent to 500 students who are studying food and nutrition in three colleges and three universities located in Seoul and its suburbs. Data from 490 usable responses were analyzed using the SAS package program. The study showed that although the students have a favorable perception about hiring older workers, they prefer to hire younger people under the age of 30 father than older workers(over 55 year old). Additionally, even though both the younger and older people have the same ability, they would still choose to hire younger people. Also, they think that older workers are harder to train, find it more difficult to adapt to new ways of working and are slower in their work. On the positive side, the response did show that students realize that older workers are not absent from work as frequently as younger workers and that they are more cooperative than younger workers are. The data also showed that the students who had a more positive experience with older workers also perceive those workers in a more positive way than the students who did mot have experience with older workers(p<0.0001). Finally, students who have family members who are older workers(p<0.01) and students who are planning to work past fifty-five years of ages show more positive attitudes toward older workers(p<0.01). Moreover male students showed more positive attitudes toward older workers than female students did(p<0.0001).

The Dietary Characteristics of Middle-aged People by BMI and food Habits

- This study was conducted to compare the dietary characteristics of middle-aged people with different obesity index or food habits. Questionnaires were collected from 480 subjects in Daejon. The data were analyzed by chi2-test and ANOVA. The subjects were distributed into the normal weight group(66.3%), overweight group(18.3%) and underweight(15.4%) based on BMI. Twenty-five percent of males and 23.0% of females in the 50's belonged to the overweight group. The least consumed food was milk or milk products among the five basic food groups. The frequency of exercise was low in their daily life. The had better food habits than the males. The overweight group had worse food habits than the normal weight or underweight group. The underweight group ate light meal and ate slowly. The better the subjects had, the more regularly they took breakfast(p<0.001). The good food habits group were more interested in their dietary life and were more careful in selecting foods for their health than the bad food habits group(p<0.001). Based on the results of this study, the practical education programs should be focused on more intake of milk or products and on daily exercise for the self-health maintenance of middle-aged people. Also, proper education programs should be developed for males and females in their 50's to practice weight control and regularity of meals in their life, and to utilize nutrition knowledge in dietary life. Further research on the comparision of dietary characteristics are needed with consideration of the subjects ratio according to BMI.

- This investigation studied the relationship between Body Mass Index(BMI) and dietary intake, levels of serum lipid, lipoprotein(a) and plasminogen activator inhibitor-1(PAI-1) of 28.449 Korean adults(16.937 men, 11.542 women) from 1995 to 1999. The dietary assessment was conducted using 24-hour dietary records and food frequency questionnaires. During this five year study, the BMI normal-weight group, as a percentage of the annual test population, decreased from 68.0% to 60.2%, while the BMI overweight and obese groups collectively increased from 25.0% to 29.7%. The levels of serum total cholesterol (TC), LDL-cholesterol(LDL-C), Triacylglycerol(TG) and PAI-1 increased as the values of BMI increased, while the levels of HDL-cholesterol(HDL-C) and lipoprotein(a)(men only) appeared to decrease as values of BMI increased. The levels of daily energy intake also increased as values of BMI increased in both men and women. The obese group had significantly higher levels of carbohydrate, protein, fat(men only), and cholesterol intake than those of the normal-weight, underweight, and overweight groups. In men, BMI positively correlated with the levels of macronutrients and cholesterol intake(p<0.001), % energy of protein, fat, and alcohol intake(p<0.001), and the levels of serum TC, LDL-C, TG and PAI-1(p<0.001), while BMI negatively correlated with % energy of carbohydrate intake, and the levels of Lp(a) and HDL-C(p<0.001). In women, BMI negatively correlated with level of cholesterol(p<0.01), fat(p<0.001), alcohol intake(p<0.05),% energy of fat (p<0.001), % energy of alcohol intake and level of and HDL-C(p<0.001). Subjects who had serum TC, LDL-C, HDL-C, and TG levels greater than the standard reference values(TC>240 mg/dl, LDL-C>130 mg/dl, HDL-C>35, TG>200 mg/dl) exhibited a higher intake of the three macronutrients, iron, calcium, meat, milk and fatty foods than those subjects who had serum lipid concentrations less-than-or-equal-to the standard reference values. Overall, there was positive correlation between the high risk factors of vascular disease variables, dietary intake, and BMI. Prevalence of hypertension and high blood sugar were increased as BMI increased, but the prevalence of hypertension is decreased as the year goes by. These findings showed that dietary intake, level of serum lipids and other vascular disease risk factors increased as BIM increased. Therefore, middle or upper class Korean adults who have high BMI should improve their eating habits. This involve reducing alcohol, animal fat, high carbohydrate foods, and overall food intake, and balancing intake in order to lower vascular disease risk factors, including obesity.

- Intake of soy protein the fisk factors associated with cardiovascular disease in postmenopausal women. This study was designed to effects of isoflavone supplementation on serum lipids in 16 hyperlipidemic postmenopausal women . For this purpose, an intervention study was conducted for 12 weeks. Subjects were healthy, free-living women consuming habitual diets with 0.3g/d of isoflavone. Food and nutrient intake was obtained by 24-hr recall method and anthropometric measurement were made. Systolic and diastolic blood pressure, total serum cholesterol. HDL-cholesterol and LDL-cholesterol were determined before and after the isoflavone supplementation. The results were summarized as follows. The average age, hight, weight and BMI of the subject were 65.3 years, 151.4 cm, 62.2 kg and 27.1, respectively. The systolic blood pressure and diastolic blood pressure were not reduced significantly with isoflavone supplementation. Total cholesterol (p<0.001), HDL-C(p<0.05), and LDL-C(p<0.01) were significantly increased after isoflavone concentration. In conclusion, isoflavone supplementation was not effective to modify risk factors for cardiovascular disease.

- This study was designed to determine serum osmolality and to investigate the association among serum osmolality, health-related factors and biochemical indices. Two hundred thirty seven elderly(86 male, 151 female : mean age 73.8) residing in the chungbuk area participated. Sociodemographic data and self-perceived health status were obtained by interview, and biochemical parameters were measured. The mean serum osmolaity was 301.2+/-10.1 mOsm/ kg(range 240.8~328.9 mOsm/ kg) and serum osmolality of the females(302.4 mOsm/ kg) was significantly(p<0.05) higher than that of the males(299.0 mOsm/ kg). Only 11.0% of the elderly were within the accepted normal range(280~295 mOsm/ kg) and the majority of the subjects were in a hyperosmolar state(60.8% : 296~307 mOsm/ kg). It was observed that serum osmolality was reduced with increasing age but not statistically significant. There was no significant difference in serum osmolality according to marital status. Serum osmolality was significantly lower(p<0.05) in the elderly with higher self-perceived health scores than the elderly with lower self-perceived health scores, however no significant difference was observed between the presence or absence of disease. Serum osmolality tended to be higher(p=0.06) in the elderly with difficulty in mobility than the elderly who have no problem in mobility. Serum osmolality tended to be higher(p=0.06) among drug users compared to that of non-users. There was a significant association between serum osmolality and serum albumin in females but not in males. Significant associations were shown between serum osmolality and serum transferrin in both sexes. However, there was no significant correlation between serum osmolality and hemoglobin or hematocrit except with hematocrit in females only. The results of this study indicated that the elderly were dehydrated and hydration state seems to influence health status and, the elderly should be encouraged to drink an adequate amount of fluids. Also, the hydration state is required to be assessed and considered for the interpretation of biochemical parameters.

- The purpose of this study was to investigate teacher's perceptions about the nutrition education of elementary schools. A total of 226 elementary school teachers within Changwon and Milyang city participated in this study. The results of this study were as follows. 90.8% of the teachers responded that nutrition education is necessaty in the elementary schools, and positive responses in the Milyang urban area were higher than that of the Changwon rural area(p<0.001). The main reason for the necessity of nutrition education was for proper growth and good table manners and the Milyang urban city was significantly higher than three other classes in 'for proper growth'(p<0.05). The starting period for nutrition education was supported by 72.1% of the teachers as beging when the children are in kindergarten, proving that this early nutrition education is supported. Especially in the Changwon urban support was higher than in the three other classes(p<0.01). The perceptions about a suitable person to teach nutrition indicated parents(39.6%), dietitions(35.5%), class teachers(24.9%), and the beliefs about nutrition specialists or dietitions was very low. 53.6% of the teachers responded that they would not teach nutrition education themselves and the main reason for this was that believed the teacher's work would be too hard(32.0%), lack of nutrition knowledge(29.4%), and that such courses should be taught courses by specialists(29.4%). However, 51.6% of the teachers disagreed with having a separate course for nutrition education. Proper eating habits(62.7%) and growth and nutrition(28.6%) were the main categories within nutrition education. The teacher recognized that elementary students' nutrition problem is to eat too much processed and instant foods(39.1%), deviated foods(36.9%), and lack of table manners(19.4%). The most effect method was to link the subject matter with the parents(38.7%), use of audio visuals and education materials(28.3%), and need of nutrition education teachers(10.1%) was low.

Determinants of Nutrition Service Utilization in Health Centers

- The purpose of this study was to identify the determinants of nutrition service utilization in health centers. Utilization of nutrition services for chronic discase were also investigated. Interview survey using questionnaire was conducted to collect the data required for analysis. The study subjects were 300 residents over 40 years of age, and 15 health workers representing health center service personnel in Kypnggi province. A wilcoxon 2-sample test and Kruskal-Wallis test were used to compare the distribution of health centers by health center characteristics and nutrition services. A chi-square test was used to test the association between service utilization and personal variables of the population. A multiple logistic regression analysis was used to measure the relative importance between the variables on service utilization. The results showed that only 10.0% of the study subjects used nutrition services provided by the health centers. Pilot project implementation and location of health centers, as well as educational level of the residents were significant factors influencing the utilization of the nutrition services provided by the health centers. Among the variables, pilot project implementation was the most important factor that influenced the nutrition service utilization in health centers.

- The clustering of insulin resistance with hypertension, glucose intolerance, hyperinsulinemia, increased triglyceride and decreased HDL cholesterol levels, and central and overall obesity has been called syndrome X, or the insulin resistance syndrome(IRS). To develop a nutrition service for IRS, this study was performed to evaluate the prevalence of each component of the metabolic abnormalities of IRS and analyze the clustering pattern of IRS among subjects living in the Taegu community. Participants in this study were 9234(mean age ; M/F 48/40yrs);63.5% were men, 24.4% were obese, 13.3% had hypertension. 3.7% had hyperglycemia, and 32.4% had hyperlipidemia. The IRS was defined as the coexistence of two or more components among metabolic abnormalities; obesity, hypertension. hyperglucemia and hyperlipidemia. The prevalence of IRS in Taegu was 19.2%(M/F:20.8%/16.4%), the clustering of these fisk variables was higher in advanced age group. Among the subjects of IRS having two of more diseases, 75.6% were obese, the pattern were similar in men and women. The younger, the higher the prevalence of obesity associated clustering patterns. The prevalence of obesity associated patterns among the hyperglycemia associated clustering patterns was 44.5%. The samples of the representative clustering patterns were obesity and hyperlipidemia (8.0%), hypertension and hyperlipidemia(3.2%), hypertension, obesity and hyperlipiemia(3.1%), hypertension and obesity(2.3%), and hyperglycemia and hyperlipidemia(0.8%). The clustering of obesity and hyperlipidemia until 50 year old groups, and the clustering of hypertension and hyperlipidemia in the 60 and 70 age groups were the most prevalent. We concluded that insulin resistance syndrome was a relatively common disorder in the Taegu community, and prevalence and the characteristics of the intervention strategies for IRS are desired, an effective improvement will be achieved.
