Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans

This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . Blood Pressure Distribution of Adolescents in Taejon City and its Relationship with Obese Index and Several Environmental Factors

Blood pressure distribution of adolescents and its several related factors including obesity were investigated through blood pressure measuring by automated oscillometric method, anthropometry, and questionnaire. Subjects were 480 female and 480 male middle school students living in Taejon. Mean values of systolic(SBP)/diastolic(DNP) blood pressure of male and female students were 118.4$\pm$11.0/69.5$\pm$9.6 mmHg and 116.9$\pm$10.4/69.5$\pm$8.6 mmHg, respectively. Their blood pressures, particularly SBP, increased with age and showed higher value in the males than in the females. All frequency distributions of SBP and DBP, by sex and age, showed normal curves and their 50th, 90th, and 95th percentiles were presented. As obese index increased, both SBP and DBP were elevated and the prevalence of hypertension which was classified by Task Force Report(1987) increased, particularly in male students. Body weight and BMI were positively correlated with both SBP and DBP, and hight was also positively correlated with SBP or DBP. Our data inicated that blood pressure and hypertension prevalence of adolescents tended to increase and various internal and external environmental factors affected them. It was emphasized that blood pressure measurement should be done in every medical examination of adolescents and the nationwide Korean standard of blood pressure for adolescents have to be prepared. Study on the Food Habits of College Students by Residences

This study was carried out with 1030 students of Seoul National University to investigate the relationship between residences and food habits of college students. The results obtained were as follows. 1) Food habit scores of the students living in dormitories and in boarding homes with cooking facilities were significantly lower than those of other students, especially with regards to the balance of five basic food groups and skipping breakfast. 2) Energy intake of students living in the boarding homes with meals were low. Students in the boarding home also showed low intake of calcium and iron, whether meals were provided or they had cooking facilities. 3) Cafeteria meals on campus showed better nutritional values than meals at home, or restaurants around the campus. But students were not satisfied with campus cafeterias, because the food items were monotonous, they had long waiting service lines, preferred foods were limited, dishes were not clean, and stated of hygiene did not seem satisfactory. Nutrition education for college students is needed, and menus of campus cafeterias should be developed to meet the various needs of students. (Korean J Community Nutrition 1(2) : 189~200, 1996)

Ecological Studies of Maternal-Infant Nutrition and Feeding in Urban Low Income Areas

This study was aimed at evaluating the lipid status of 122 pregnant women, who were attending peripheral community clinics in low-income areas and were vulnerable in terms of public health, by anthropometric measurements, estimating dietary intakes and analyzing serum lipids content and fatty acids composition. Regarding the skinfold thickness, it has increased more significantly in suprailliac than in triceps, biceps, subscapular, which means body fat has been accumulated in central parts more than in distal parts of the pregnants. It is found that intakes of energy, protein, lipid and carbohydrate are desirable while calcium, zinc and copper are low and sodium is excessive. Regarding dietary fat intake, the energy $\%$ of fat was within the same range as RDA(20%) for adults, but energy $\%$ intake from each fatty acid was usually low, especially energy % of monoun-saturated fatty acid intake was below RDA for adult women. And intake ratio of $\omega$6$\omega$3 fatty acids was desirable. Accordingly, it is thought that dietary fat intake was balanced in quality. Serum lipids concentrations were increased with the length of pregnancy and were higher as compared with non-pregnant women. Polyunsaturated fatty acids, especially $\omega$6 fatty acids, have gradually decreased with the length of pregnancy. Serum lipids content and fatty acids composition were not greatly influenced by dietary fat but serum free fatty acid level was negatively related to energy, carbohydrate, fatty acids and cholesterol intake. On the basis of the above results, it can be predicted that lipid metabolism for mother and fetus is changed by pregnant status and multilateral research on maternal nutrition in terms of areas and income levels will have to be made. Nutritional and Health Status of Korean Elderly from Low-Income, Urban Areas and Improving Effect of Meal Service on Nutritional and Health Status - II.Biochemical Nutritionsl Status and Health Status-

This study was conducted to assess biochemical nutritional status and health status of 53 men and 130 women aged over 65 years and residing in low-income areas in Seoul. Nutritional status and health status were determined by hematological analysis, incidence of disease, and clinical symptoms. Mean serum cholesterol levels were 159.7mg/dl in men and 182.4mg/dl in women and triglyceride levels were 124.6mg/dl in men and 130.6mg/dl in women. The serum lipid levels were higher in women than in men, and 88% of the subjects belonged to normal range and 12% belonged to hyper lipid. The mean hematological indices.; Hb, Hct, RBC, MCH, serum iron belonged to normal ranges. The proportion of anemic state were 16~18% in men and 20~25% in women. Mean serum Ca, Cu and Zn levels were 9.4mg/dl, $84.2\mu\textrm{g}/dl$ and $75.8\mu\textrm{g}/dl$ for male and $9.5\mu\textrm{g}/dl$, $88.3\mu\textrm{g}/dl$ and $75.6\mu\textrm{g}/dl$ for female, respectively. However, proportion of low Ca, Cu and Zn status were 8~20%, 77~84% and 12~34%, respectively. The biochemical indices were not significantly different according to age, BMI, obesity rate. Cholesterol, RBC, MCH and serum iron levels were significantly different between men and women. (Korean J Community Nutrition 1(2) : 215-227, 1996)

Nutritional and Health Status of Korean Elderly from Low-Income, Urban Area and Improving Effect of Meal Service on Nutritional and Health Status -III. The Effect of Meal Service on Protein Nutrition Status and Serum Lipids-

This study was designed to assess the effect of a meal service for home-staying urban elderly people with low-income on their protein nutrition status and serum lipids. One hundred and eighty three subjects, who had already completed the first nutritional survey were assigned to two groups : meal served(served) and non-meal served (non-served). A meal approximately containing one half of the RDA for energy, protein, calcium and iron was served as lunch everyday to served group. After 6 months of meal service, the second nutritional survey was done and changes of parameters were analyzed with paired t-test. Served females showed significantly increased intake of energy while non-served females showed significantly decreased intakes of energy, protein and fat after 6 months. Served males were observed with significantly higher intake of energy, fat, vitamin A, niacin and ascorbic acid, whereas intake of vitamin A was lower in non-served males. Arm circumference of both served and non-served females were significantly lowered after 6 months. Mean DBP of served males was significantly lower, while SBP of served females was significantly higher after 6 months of meal service. Serum total protein, serum albumin, serum cholesterol and LDL-cholesterol were significantly increased within normal range regardless of gender and meal service. But HDL-cholesterol was significantly higher in only served elderly subjects after 6 months. (Korean J Community Nutrition 1(2) : 228-238, 1996)

Applying the Theory of Planned Behavior to Understand Milk Consumption among WIC Preagnant Women

Despite the importance of prenatal nutrition, many studies find inadequate calcium intake among pregnant women. The purpose of this study was to investigate the value of the Theory of Planned Behavior in explaining the intentions and the actual consumption of milk among pregnant women participating in or eligible for WIC. A cross-sectional survey was conducted to collect information regarding attitudes, subjective norms, perceived control, milk allocation within the family, intentions and consumption of milk. The survey questionnaire was developed using open-ended questions and interviews with 112 pregnant women. One-hundred-eighty women recruited from prenatal clinics completed the survey questionnaire. Multiple regression was used separately to investigate the association of factors to intentions and to the consu-mption of milk, as proposed in the theory. Milk allocation within the family was used as an exploratory variable to explain milk consumption. Study findings revealed that all three factors, attitudes, subjective norms and perceived control contributed to the model in explaining intentions (explained variance : 36.2%), with perceived control being most important. For milk consumption, intentions and perceived control were related significantly to actual consumption, while milk allocation within the family was not (explained variance : 44.6%). These findings suggest that perceived control is important in understanding both intentions and milk consumption, providing empirical evidence for the Theory of Planned Behavior. With respect to the role of perceived control, more strong evidence was provided in explaining intentions. Findings suggest that educational interventions to increase milk consumption among pregnant women should incorporate strategies to enhance the perception of control, and to strengthen positive attitudes and to elicit social support from significant other. Dietary Behaviors, Consumed Time for Job Duties and Fatigue Levels of Dietitians by School Foodservice Type in Taejon and Chungnam

The school foodservice(FS) dietitians of conventional, co-management and central kitchen systems were compared for their levels of fatigue degrees in order to examine their work specification but no to diminish their educational roles. One hundred and thirtheen primary schools in Taejon and Chungcheongnam-Do participated in the survey, of which foodservice were consisted of 99 conventional types, 7 comanagement types and central kitchen types. Information was collected about the various working hours dietary behavior and fatigue symptoms of the dietitians. The results of this study were as follows : Among 3 school foodservice types, dietitians spent 8 hours and 16 minutes daily for their for their work on the average. However, the central kitchen type dietitans in cities spent 3 hours more compared to the others. The data from the dietitians' work schedule sheets showed that the 5 parts of dietitian's duties set in the school foodservice laws were not well performed. In the central kitchen type they spent more time for food production whereas in the conventional they spent more time for menu formation and hygiene control. It was found that fatigue levels of school foodservice dietitians were chronic with no difference among foodservice systems or among school locations. Our results indicated that nutrition education in primary schools was almost neglected. It is suggested that the school foodservice should meet all the objectives of school-lunch program even though they saved dietitians' work load more or less. The importance of students to improve their dietary habits could never be overlooked. (Korean J Community Nutrition 1(2) : 250-259, 1996)

Development of Experimental Animal Model with Duodenal Ulcer for Nutritional Study and its Nutritional Characteristics

This study aimed to develop an experimental animal model of duodenal ulcer for application to nutrition, and to verify the nutritional characteristics. The 7 different doses and 8 different injections of cysteamine and 2 kinds of diets with different casein levels(10%, 20%) were tested to examine the incidence and the process of duodenal ulcer in female Sprague-Dawley rats. The result showed that the duodenal ulcer was induced when rats fed 10% casein diet(for 9 days) were injected 6 times with 13mg/100g BW cysteamine. The first injection was conducted after 24 hours of fasting, followed by the 2nd shot after 3 hours. The following shots were repeated every 3 day(4th and 7th days). Duodenal ulcer was observed 3 days after the last injection by optical microscope and the naked eye. The characteristics of nitrogen bioavailability of duodenal ulcer model were as followings : 1) gastric emptying rate was faster, 2) trypsin activity at duodenum was higher, 3) total protein concentration at serum and nitrogen retention rate were lower than the control.
