Nutritional Status of Preschool Children in Low Income Urban Area: Anthropometry and Dietary Intake

- The nutritional status of 125 preschool chidren(Female : 56, Male : 69) residing in low income area of Seoul was surveyed. The mean family size was 4.3 and the mean monthly income was 921,000 won which was below the poverty level. Average heights of boys aged 3, 4, 5 and 6 were 98.6 cm, 106.1 cm, 111.9 cm and 116.0 cm and those for girls were 99.4 cm, 106.4 cm, 110.9 cm, and 116.0 cm, respectively. There was no significant difference between the height of boys and girls. The proportion of children showing stunted growth(<90% of Korean standard of height) was 3.2%. The mean weight of boys for each age group(3, 4, 5, 6) were not significantly differient from that of girls. The proposetion of children assessed as moderately underweight (<80-90% of Korean Standard of weight) was 10.4% and 3.2% of children was underweight(<80% of weight standard). The group of children aged 3 were taking adequate energy, whereas the mean energy intake of children aged 4.5 and 6 were wirthin 77.3-78.6% of RDA. The proportion of energy derived from carbohydrate, fat and protein was 60.1%, 24.6% and 15.3%. The high proportion of energy derived from fat seems partly due to high energy intake from fat in the snack. The nutrients which did not meet 75% of the RDA were vitamin A and calcium for children aged 5, and iron for children aged 3 to 5. Carbohydrate intake was positively correlated with the height, weight and girth of chest(P<0.05-P<0.001). There was positive correlation between protein intake and weight, girth of chest and BMI, respectively(P<0.05). Iron intake showed positive relationship with height, weight and BMI(P<0.05).

- The purpose of this study was to determine biochemical parameters related to protein and immunity. Subjects were 125 preschool children(M:69, F:56) residing in low income area of Seoul. Mean serum total protein of the children aged 6 was 7.3g/dl which was significantly higher than 6.6g/dl of the group aged 3. The mean serum albumin was 4.7g/dl for 3, 4, 5 age group, and 4.9g/dl for 6 age group and there was no significant difference. Serum retinol binding protein(RBP) is used as a sensitive indicatior of protein, becase it tends to fall rapidly in response to protein status and respond to quickly dietary treatment. Mean RBP for each group(3, 4, 5 and 6 age group) were 2.5 microgram/dl was 15.9%, 19.2%, 24.3% and 16.7%, respectively. The 24-hour urinary excretion of hydroxyproline was 7.9 mg, 14.6 mg, 11.7 mg and 11.8 mg for each group and the mean excretion of all children was 12.2 mg/day. Children aged 3 were excreting significantly lower amount of hydroxprolinc per day than the children aged 4. The mean hydroxyproline index were 2.18, 2.39, 2.52, 2.80 for each age group and the mean of a group aged 6 was significantly higher than that of the group aged 3. The proportion of children assessed as malnourished and impaired growth(hydroxyproline index<2.0) was 18.8%, 4.9%, 2.5% and 4.3%, respectively. The nutrients which showed significant relationship with protein and immunity parameters were niacin, vitamin C and calcium. Vitamin C showed significant positive relationship(p<0.05-p<0.01) with serum RBP, total protein and globulin. The triceps skinfold thickness was significantly and positively correlated with serum globulin. Serum IgG showed significant positive relationship with height, weight, girth of chest and midarm circumference(p<0.05-p<0.01).

- This study was designed to assess the iron nutritional status of girls at puberty in Kangnung area. The subjects consisted of 161 adolescents in sixth-grade in primary school and first-grade in middle school girls. Anthropometric measurements were taken for body weight, height, percentage of body fat, and circumferences of waist and hip. Nutrient intakes were assessed by modified 24-hour recall method. Food models and other measuring tools were also used. Fasting blood samples were obtained and analyzed for hemoglobin(Hb) concentration, hematocrit(Hct), serum iron(FE) and total iron binding capacity(TIBC). Mean values for Hb, Hct, Fe, TIBC, TS and serum ferritin were 13.6+/-0.9g/dl, 39.6+/-3.9%, 91.3+/-36.3 microgram/dl, 327.9+/-45.2 microgram/dl, 28.3+/-11.8% and 37.4+/-24.2ng/ml, respectively. Prevalence of iron deficiency greatly varied by indices from 4.8% when judged by Hb to 18.4% by serum Fe concentration. The Hb concentration was positively correlated with Hct(r=0.641), serum iron(r=0.266) and transferrin saturation(r=0.237)(p<0.05). On the other hand, serum ferritin concentration showed significantly negative correlation with TIBC(r=-0.572). Mean daily intake of iron was 14.94mg and heme iron intake was 1.13mg and which was 7.6% of total iron intake. Total absorbable iron calculated by the method of Mosen was 1.38mg and bioavailability of diectary iron was 9.3%. These results suggest that the prevalence of iron deficiency of pubertal girls is very high, therefore the guidelines for diet and social supports, such as, school food service system should be provided to improve their iron status in middle school students.

- This study was due to investigate the dietary intakes, serum lipids and other hematological indices in female adolescent smokers. The subjects were 85 smokers, whose average pack-year(smoking years on the basis of one pack of cigarettes per day) was 1.26 and 87 nonsmokers who were female high school students in Seoul. An anthropometric measreument was performed and % body fat was also analyzed by the Bioelectrical Impedance Fatness Analyzer(GIF-891). Dietary intakes and food habits were examined through questionnaires and nutrient intakes were analyzed by the Computer Aided. Nutritional analysis program for professional(CAN-pro). Serum TG, HDL-cholesterol and total-cholesterol levels were measured with test kits. Serum glucose, albumin, GOT and GPT were measured by automated dry chemistry system, SPOTCHEM 4410. Hemoglobin, hematocrit, RBC, WBC and MCV were determined by Semi Automated Microcell Counter(F-520). All data were statistically analyzed by SAS PC package program. There was no significant difference in the anthropometric measurements between smokers and nonsmokers. The caloric intake in adolescent smokers tended to be higher than that of nonsmokers but the difference was not statistically significant. In addition, there was no significant difference between smokers and nonsmokers in biochemical indices. Analysis of serum lipids showed that the serum levels of total-cholesterol and LDL-cholesterol of nonsmokers were unexpectedly significantly higher(p<0.05) than those of smokers. Overall results indicate that smoking itself with short pack-year in healthy female adolescent did not seem to influence apparent health and nutritional status.

- This study was conducted to identify the determinants of regional body fat distribution of obesity(upper body obesity and lower body obesity) for adolescents. The macronutrient consumption pattern utilized the most important variables to test for potential determinants. A total of 726 adolescents living in rural areas in Korea had been observed for four years from 1992 to 1996 about their diet, sexual maturation, serum components and physical growth. The study design was similar to that fo a casecontrol study. Logistic regression analysis were used as an analytical method to identify the determinants of upper body obesity and lower body obesity. Odd ratios were estimated from the regression to identify the determinants of upper body obesity and lower body obesity. Odd ratios were estimated from the regression to identify the risk factors. Fat consumption pattern was the most frequent one among the three macronutrient consumption pattern of carbohydrate, fat and protein. Prevalence of obesity for the subjects was 9.5%. Prevalence of upper body obesity was higher in malestudents than in female students. On the other had, prevalence of lower body obesity was higher in females. The results of the logicstic regression analysis showed that the risk factor for upper body obesity was sexual maturity rather than dietary factors. None of the factors included in the analysis for lower body obesity appear to be the risk factor. The result may suggest that to develop a determinant model for obesity of adolescents, the model should include a wider range of variables other than diet, sexual maturity and changes in blood serum.

An Assessment of Obesity and Dietary Habits of College Students Taking the Course Health and Diet

- We examined the obesity levels and dietary habits of 323 college students taking the course 'Health and Diet' as a part of theirnutritional education. The average percentages of body fat, BMI and RBW for subjects in this study were 16.7%, 21.5kg/m2 and -1.2% in male students and 27.3%, 21.2kg/m2 and 1.0% in female students, respectively. BMI and RBW indicated fewer cases of obesity among female students than BIA did. The average intake of energy by subjects was lower than the RDA of energy. In particular, the average intakes of calcium, iron, and vitamin A by female students were much lower than the RDA of energy. In particular, the average intakes of calcium, iron, and vitamin A by female students were much lower than the RDAs of each of those nutrients. The percent RDAs of nutrient intakes of energy, protein, thiamin, and riboflavin were lower in underweight students than in students of normal and over-weight. Female students were found to skip mealsand consume sweet foods more frequently than male students. The obesity levels of students related to the number of meals eaten per day as well as the frequency of consuming sweet foods. Although, it was not possible to determine statistically, the levels obesity also seemed to correspond to the speed at which meals were eate as well as the frequency of eating salty foods. These results suggest that in order to assess obesity propertly, more research that focuses on subjects' gender should be conducted. In addition, nutritional education should emphasize the importance of intakes of energy and phosphorus for male students, and the danger of iron and calcium deficiencies for female students. We should also educate students about dietary habits which contribute to obesity such as overeating and the intake of sweet foods.

The Effect of Daytime Jobs on the Dietary Behaviors and Nutrient Intakes among Evening Class University Students

- This study was conducted among the university students to investigate the effects of working in an daytime and studying in an evening class on dietary behaviors and nutrient intakes. A 24-hour recall method was used along with food frequency questionnaires. The subjects were composed of three groups : 172 were in the daytime calss regular student group(RS), 178 were evening class students without a job(ESWOJ), and 140 were evening class students with a job(ESWJ). An average age of the ESWJ was 31 years old, which was 9 years older than the RS and 7 years older than the ESWOJ. The types of the residence were significantly different among the groups ; were seif-boarding 74% of the ESWJ stayed at home while 47% of the RS were self-boarding and 61% of the ESWOJ. The survey shows that while a great majority of the RS and ESWOJ has breakfast and supper at home, about 10% and 17% of the ESWJ ate breakfast and supper, respectively, at the cafeteria in their work place. The most frequently skipped meal was the breakfast in all groups. Approximately 9% of the ESWJ tended to skip the breakfast, while 27% of the RS and 33% of the ESWOJ did. It turns out that the mose common reason why ESWJ skipped meals was due to a lack of time. About 71% of the ESWJ ate out more than once a week compared to 57% and 62% of the RS and ESWOJ, respectively. Approximately 30% of the ESWJ ate out more than once a week compared to 57% and 62% of the RS and ESWOJ, respectively. Approximately 30% of the ESWJ spent less than 10 minutes at meal time, showing a hurried eating habit. The results show that the ESWJ tended to eat a variety of food s and eat less instant foods, but drink coffee more often compared to the other two groups. The results also show that in all groups, the intakes of energy, Ca, and vitamin A were lower than the Korean Recommended Daily Allowances(RDA). In addition to these nutrients, protein, vitamin B1, vitamin B2, and niacin were also lower than the RDA in all make students, and Fe was lower than the RDA in all female students. Among the deficient nutrients, vitamin A showed the lowest % RDA : 48% in the male and 64% in the female students. Among the deficient nutrients, vitamin A showed the lowest % RDA ; 48% in the male and 64% in the female students. The intakes of Ca, Fe, and niacin in the ESWJ were significantly greater than those in the RS and ESWOJ groups, and the vitamin C intake in the ESWJ was greater than that in the ESWOJ(p<0.01). In conclusion, university students in all three groups have a serious imbalance in the nutrient intakes, and the evening class students with a daytime job have unhealthy dietary behaviors in the light of high frequency of eating out and heavy coffee drinking, a rushed eating pattern, and therefore showing a strong need of proper nutritional education for them.

- Recent epidemiologic and clinical students have shown that plasma cholesterol and triglyceride levels are independent risk factors for coronary heart disease. However, there is not much data on the characteristics of anthropometry and lipid profiles of hypercholesterolemic patients with hypertiglyceridemia. In this study, 112 hypercholesterolemic subjects(T-C 240mg/dl) were divided into two groups by their plasma triglyceride levels. We compared the anthropometric measurements and lipid profiles of the subjects between the two groups : the simples hypercholesterolemic group(SHC, TG<200mg/dl) and the combined hypercholesterolemic group(CHC, TG 250mg/dl). The distribution of the subjects into the SHC and CHC groups was 36.6% and 47.3%, respectively. The frequency of the CHC patients decreased with age. The subjects in this group had higher weight, BMI, HWR, cricumferences of mid arm, waist, hip and thigh, and skinfold thicknesses of biceps and triceps than those of the SHC subjects. The difference of plasma total cholesterol level was mainly due to the difference of VLDL-C levels. These differences resulted in the CHC subjects having higher atherogenic indexes and T-C/HDL-C ratios than those of the SHC subjects. Also, the former had higher Apo-B and insulin levels than those the latter. However, blood pressure, fasting blood glucose and HDL-C levels were not significantly different between the two groups. These results suggest that hypercholesterolemic patients with hypertriglyceridemia have riskier lipid profiles for CHD than those of patients with normal triglyceridemia. They also indicate that CHC is closely associated with glucose resistance syndrome(obesity, hyperglycemia, hyperinsulinemia and hypertriglyceridemia), and more prevalent in young people.

- This study inrestigated the dietary behavior, nutritional intake, and health status of industrial workers. Two hundred seventy workers(165 men and 105 women) in Masan, who were provided food services were selected as survey subjects. They were given questionnaires on nutrition knowledge, food frequency questionnaires, and blood analysis were conducted through an annual health examination from May to August 1998. The average age of the subjects was 35.7+/-11.8 years, and 80% the of women were in their teens and 20's and 80% of men were in their 30's and 40's. The nutrition knowledge score was higher in women than in men, while food habit score was higher in men than in women. As age increase, the degree of life stress became lower and food habit score was higher in men than in women. As age increases, the degree of life stress became lower and food habit was improved. In women, all nutrient intakes were avobe the recommended dictary allowances(RDA), and in men intakes of only vitamin E and calcium were under RDA. Levels of serum total cholesterol, triglyceride, and blood glucose, blood pressure, body mass index were positively related to age. The number of subjects suspected of havins chronic disease was the highest for those in their 40's. The results indicate that nutritional status of industrial workers in the present study was fairly good probably due to food services, but their health status should be supervised as their age increases.

- The increased number of elderly in Korea presents great challenges for community nutrition programs. Strategies to enchance elderly nutrition are needed, as well as dietary advice that are reality-based and tailored to the need of the elderly. The purpose of this study was to evaluate the overall effectiveness of an elderly nutrition improvement program(ENIP), by assessing changes in nutrition knowledge, attitudes, and behavior after program completion. ENIP was conducted in Suwon for 4 months in 1998 at 5 centers for the elderly. The program provided individual nutrition counseling and brief biweekly group education sessions. To stimulate participation and motivation, the elderly in two centers were served free milk packs(200ml) three time a week during the program(milk+education group), and the elderly in 3 other centers were served yoghurt on the program day(education group). Altogether, 102 subjects finished the program(milk+education group 67 ; education group 35). Data about nutrition knowledge, dietary attitudes, diet records(24 hour recall) and dietary habit was collected before(baseline) and after the program(follow-up). The Drop-out rate for the milk+education group(2.9%) was significantly lower than that for the education group(30.0%)(p<.001). The Nutrition Knowledge fo females milk+education group increased significantly after the program, but no significant differences were observed in other groups. Over 60% of all the elderly showed positive responses to the healthy eating attitudes. Energy, protein, vitamin A, riboflavin, Calcium and Potassium intakes improved in the male milk+education group. The Vitamin C intake improved in the male education group. Energy and vitamin A intakes improved in the female milk+education group and energy, vitamin A, thiamin, riboflavin, niacin, Ca nad P in the female education group. A stepwise multiple regression analysis was performed to examine the effects of food intake changes on overall nutrient quality. For all elderly, the fish intake explained 12.6% of the variance, followed by candy intake and vegetable intake(model R2=19.6%). Dietary habits such as meal time regularity and salty food reduction were markedly improved in both groups. Sixty-one percent of the milk+education subjects stated their participation was voluntary and active, while 51% of education subjects did. Sixty four percent of the subjects stated their interest on diet increased remarkably through program participation. Considering the results, it is conjectured that ENIP had a great impact on nutrient intake, dietary attitudes and habits. Milk supplement showed no direct effects on elderly nutrition improvement, but it encouraged the program by reducting drop-out rates and inducing voluntary participation. Therefore milk supplement could enhance the community nutrition education program. Moreover it is strongly suggested that the ENIP be expanded to other communities.

- The purpose of this study was to find out the common characteristics of eating behaviors of the long-lived elderly in order to offer the basic data for establishing dietary guidelines for a long and healthy life for the general population. The subjects were three-hundred elderly people over age 85 living in Kyungpook Sung-ju who have no problems in daily living. The general characteristics, nutrition knowledge and attitudes, meal patterns, and food preferences of the subjects were surveyed by individual intervies. The levels of the nutrition knowledge of the subjects were very low(average score ; 4.3) and the correlation coefficient between their nutrition knowledge and attitude score was also low(r=0.323, p=0.000). Most of the subjects(93%) had a regular meal pattern consuming three meals a day. The major staple food was rice mixed with other grains(75%) and the number of side dishes was mostly under four(99%). A majority(73%) had no habit of overeating and 51% of the subjects were taking snacks besides regular meals. Thirty one percent of the subjects had drinking habits and 80% of the drinking subjects had over 40 years of drinking history. The most common frequencies for intakes of various food groups were ; more than once a day(95%) for vegetables ; 2~3 times a week(74%) for green and orange color vegetables ; 2~3 times a week(72%) for fruits ; once a day(70%) for fish, eggs and legumes ; 2~3 times a week(49%) for milk ; 2~3 times a week(72%) for fruits ; once a day(70%) for fish, eggs and legumes ; 2-3 times a week(49%) for milk ; 2-3 times a week(85%) for seaweeds and 2-3 times a week(81%) for foods cooked with oil. The most preferred foods by the subjects were white rice(staple foods), soybean paste soup(soups), beef and eggs(meats and eggs), yellow croaker and hair tail(fish and shellfish), yoghurt(milk and milk products), all vegetables except carrot(vegetables), and watermelon(fruits). The popular food preparation methods included mixing with seasonings and the most preferred taste was sweet. The results showed that the eating pattern of long-lived elderly was characterized by regularity, simplicity, and no overeating.

- This study was done to investigate the 12 college volleyball player's food behavior, nutrient intakes, and their serum lipid levels during the periods of training and detraining and to provide basic data for an effective progrm for the volleyball players during the period fo detraining after the game season. The results were summarized as follows : The total daily energy intake of the players was 3,363+/-339kcal and3,692+/-499kcal during the periods of the training and detraining, respectively. During the training period, daily intakes of protein, calcium and vitamin A, and B2 were lower than the recommended dietary allowance for the players. During the detraining period, the players' daily intakes of calcium, iron, and vitamin A were even lower. However, the consumption of alcohol was dramatically increased during the period of detraining. The levels of serum triglyceride and very low density lipoprotein were significantly increased during the detraining period. Basal metabolic rates were significantly increased after the training period and diastolic pressure was decreased during the detraining period. In conclusion, out results suggest that the detraining of volleyball players for 10 days after intensive training, negatively affects their food behavior and serum lipid concentrations. Therefore, for the improvement of performance for the next game season, a planned program for the players' detraining period should be developed.

- Six-week-old Sprague Dawley rats were fed the diets of 20% casein or soy protein. Two weeks after the feeding, hepatocellular chemical carcinogenesis was initiated by diethylnitrosamine(DEN), and promoted by the diet containing 0.01% 2-acetylamino-fluorene(AAF) and two-thirds partial hepatectomy(PH). The animals were sacrificed at 8 weeks after the DEN injection. The area of placetal glutathione S-trnasferase(GST-P) positive foci, the activities of several enzymes in cellualr antioxidant enzyme systems and glucose 6-phosphatase were determined to investigate the mechanism of the anticarcinogenic effect by the dietary proteins. In another set of experiments, the drinking water of rats fed casein was supplemented with 1.5% inositol hexaphosphate(InsP6) to elucidate whether it has the comparable anticancer action of soy protein. The area and number of GST-P positive foci in the soy protein group were significantly(p<0.05) lower than those inthe casein group. The livers of rats fed casein showed moderate fattydegeneration and larger hyperplastic nodules than those of rats fed soy protein. In another set of experiments, the area and number of GST-P positive foci in the rats fed casein supplemented with InsP6 were not significantly different from those in the rats fed casein or soy protein. The lipid peroxidation of rats fed different protein sources showed no significant difference. Glutathione S-transferase(GST) activities were increased significantly(p<0.05) by carcinogen treatment in all dietary groups. Glucose 6-phosphatase(G6Pase) activities were decreased by carcinogen treatment, and hence showed a reverse relationship(r=-0.695, p<0.01) to the GST-P positive foci. Therefore, the activities in the rats fed casein were lower than those in the rats fed soy protein. These results suggest that the soy protein seems to be more anti-carcinogenic than casein by decreasing the preneoplastic lesion and by increasing the membrane stability but inositol hexaphosphate, a component of soy protein, may not be protective against hepatocarcinogenesis.
