A Comparative Study on nutrient Intake, Anthropometric Data and Food Behavior in Children with Suboptimal Iron status and Normal children

- This study was performed to investigated the difference in the nutritional status of normal children and children with suboptimal iron status. Two hundred and sixty children from 5th grade were divided into 2 groups(normal group and suboptimal group) according to the hematologic parameters of iron(RBC count, hemoglobin, serum ferritin). Normal group was composed of 71 male and 81 female and suboptimal group was consisted of 65 male and 43 female. Fat percentage of children from suboptimal group was 18.9%, which was significantly lower than 22.1% of normal group(p<0.05). TST and MAC of suboptimal group were also lower than those of normal group(p<0.05). Mean intakes of energy, protein, thiamin, riboflavin, iron were lower than those in normal group(p<0.05). suboptimal female students showed 1197.6 kcal of energy intake(63.0% of RDA) and 0.56 mg of thiamin intake(56% of RDA). Mean RBC count, hematocrit, hemoglobin, ferritin(p<0.01) and FEP(p<0.05) of suboptimal group were lower than those of normal group. Thirty-nine point seven percent of children from suboptimal group was observed with having gastrointestinal disease which was significantly higher than 22.1% of normal group.

- This study was done to investigate the effect of adolescent smoking on dietary intakes and nutritional status of serum lipids and antioxidant vitamins. Subjects were 82 somkers whose average pack-year was 0.73, and 85 nonsmokers of male technical high school students in Seoul. Anthropometric measurement was performed and % body fat was also analyzed by Bioelectrical Impedance Fatness Analyzer(GIF-891). Dietary intakes and habits were examined through questionnaires and nutrient intakes were analyzed by Computer Aided Nutritional (CAN) analysis programs. Serum TG and total cholesterol levels were measured by Spotchem sp-4410 and serum HDL-cholesterol levels were measured by test kit. serum vitamin C level was measured by 2,4-dinitrophenylhydrazine method and serum levels of vitamin A and vitamin E were measured by HPLC. All data were statistically analyzed by SAS PC package program. There was a significant difference in body fat percentage and systolic blood pressure while other anthropometric measurements were not different between smokers and monsmokers. Caloric intakes(2335 kcal) in adolescent smokers tended to be higher than that of nonsmokers (2,175 kcal) but the difference was not statistically significant. Intakes of protein(76.67g) and niacin(16.49 mg) in adolescent male smokers were significantly higher(P<0.05) than those of nonsmokers although other nutrient intakes were not significantly different. Analysis of serum lipids showed that serum HDL-cholesterol level of smokers was significantly different. Analysis of serum lipids showed that serum HDL-cholesterol level of smokers was significantly lower(p<0.05) than that of nonsmokers, whereas other lipid levels were not significantly different. Serum vitamin C level was also significantly lower(p<0.05) in adolescent smokers than in nonsmokers. In addition, serum vitamin E level, which was 7.85 mg/1 in smokers, was lower than that of 9.20 mg/1 in nonsmokers(p<0.05) while serum vitamin A level was not significantly different between the two groups. These results indicate that cigarette smoking in adolescence decreases serum levels of HDL-cholesterol, vitamin C and vitamin E even thoughth their smoking history is very short.

- The study purpose was to investigate psychosocial factors related to smoking among adolescent boys. The Theory of Planned Behavior provide the basis for the study. Twenty-five attitudinal beliefs, 9 normative beliefs and 20control beliefs were identified through questionnaire development. The data were analyzed using t-test and chi2-test. Thirty-three percent of 300 students were smokers. Most of the beliefs examined were significantly different between smokers(n=92) and nonsmokers(n=92). With respect to attitudinal beliefs, smokers responded less negatively on the items of bad health effects of smoking such as sore throat, headache, chest pain, risk of cancer and bad blood circulation(p<0.001), and decreased physical strength(p<0.05). Smokers believed less negatively on the items that smoking leads to bothering others, bad breath, yellow teeth and making them spend money(p<0.001). In contrast, smokers felt more positively on smoking as a means of stress management, relaxing, helping digestion(P<0.001). Smokers felt less pressure for not smoking from significant others. Especially mother, siblings, friends, girl friends, seniors of school were important sources of influence regarding subject's smoking. Smokers felt less confident in controlling the urge to smoke in several situations including; when they were with friends or asked to smoke by friends; after the meal, or drinking; when they were bored or stressed, upset, and when they felt unstable(p<0.001). Smokers also scored lower on specific skills to quit or control the urge to smoke as well as overall perceived control, compared to nonsmokers(p<0.001). These results suggest that interventions for adolescents incorporate diverse strategies to increase the perceived control over smoking in specific situations as well as overall perceived control, to help them realize and modify attitudinal beliefs, and to elicit support from significant others for not smoking.

Energy Value of Breakfast and Its Relation to Total Daily Nutrient Intake and serum Lipid in Korean Urban Adults

- The purpose of this study was to assess the energy value of breakfast and its relation to total daily nutrient intake and serum lipid. Dietary intakes were evaluated through the 3-day dietary recalls(interview for 1day and self-report for 2 days) from 333 Korean urban adults aged 20-49 year. Serum lipids in fasting blood samples were measured form 98 of those 333 adults. Low energy breakfast(<15% of daily energy intake) was consumed by 22.6% of males and 18.5% of females. 59.4 of males and 43.3% of females consumed a breakfast of average energy intake(15-25% daily energy intake) ; and 18.1 of males and 38.2% of females consumed a significant contribution to a total daily nutrient intake. The daily nutrient intake except crude fiber and vitamin A, B, B, and C in males and except vitamin C in females significantly increased as the energy value of breakfast increased. The differences in energy and nutrient intakes at breakfast were not made up for by other meals. To make matters worse, the satisfactory-energy breakfast group took more energy at dinner compared with the low-energy and average-energy breakfast group took more energy at dinner compared with the low-energy and average-energy breakfast groups in females. Among serum lipid parameters, TG was negatively correlated with energy provided at breakfast, and total cholesterol and LDL-cholesterol had a negative correlation with energy and carbohydrates provided at breakfast in males. For females TG was positively correlated with the ratio of carbohydrates to energy at breakfast but negatively correlated with the ratio of fat to energy at breakfast. These results suggest that satisfactory energy intakes at breakfast have positive effects on the adequacy of daily nutrient intake and may positively affect the serum lipid status.

- This study was conducted to investigate the distribution of bone density according to age and the prevalence rate of osteoporosis I 613, middle-aged women who visited Saint Bundo Hospital in Pusan from June to December, 1997. Mean bone density of lumbar spine(L2L4), and femoral neck of 50-59 years of age was significantly lower than those of 40-49 years of age(p<0.05). At the 60years of age, mean bone density of two sites were less than those of 50-59 years of age. Mean bone density of lumbar spine tin the group of sixties were 20.7% lower than that of group aged under 40 ; For femoral neck, women in their sixties showed 22.6% lower density compared to the women aged under forty. Bone density of ward's triangle of sixties were the least, which was 34.2% lower than that of group aged under 40. Bone density in lumbar spine, femoral neck, trochanter and ward's triangle correlates strongly with each other(p<0.001). The proportion of osteoporosis was 3.6% in the group of forties, 10.9% in the group of fifties and 33.8% for the group aged over 60, which was assessed by bone density of lumbar spine. Bone density of lumbar spine, femoral neck and ward's triangle were positively correlated with height, weight and BMI(p<0.001~p<0.01), and weight showed highest correlation with the bone density. Forty-four percent of variation in lumbar spine bone density was explained by age and weight.

- This study was intended to examine the zinc status and effect of zinc supplementation on the zinc nutritional status of the elderly living in the Ulsan area. The zinc intake of 207 subjects(male 97, female 110) was measured by a 24-hour dietary recall and food frequency method. Biochemical analysis were conducted from blood and urine samples to evaluate the changes of zinc nutriture with zinc supplementation. The average dietary zinc intake of subjects was 7.7+/-2.8 mg for male and 7.5+/-2.6 mg for female, which were 51.3% and 62.3% of Korean RDA respectively. The first source of zinc was cereal and grain(36%), and the second was eggs and milk group(27%). After 8 weeks of zinc supplementation, the serum zinc content was significantly increased(p<0.01), although the serum copper content was not significantly decrease. Serum HDL-cholesterol level was not significantly decreased with zinc supplementation. Serum alkaline phosphatase(ALP) activity and urinary zinc excretion were significantly increased(p<0.05). The urinary Zn/Cr was not significantly increased. It is suggested from the results that the daily zinc supplementation can be effective to improve zinc nutriture.

- The purposes of this study were to investigate the effects of drug use & the health-related habits(alcohol drinking, smoking and exercise) on the nutritional status of elderly. subjects were 362 free-living elderly aged over 65(male 131, female 231) in Chungbuk area. Interviews with questionaire on drug use, health-related habits, and nutrient intakes, anthropometric measurement and biochemical blood test were carried out from August to October in 1996. Energy intakes of the males who used drug were lower than those of males who did not use any drug, while energy intakes of the females who used drug were higher than those of females who did not. In biochemical indices, LDL-cholesterol level of the males who used drug was higher than that of males who did not have drug. serum triglycerides, VLDL-cholesterol and obesity degree(BMI & TS) of the females who used drug were higher than those of females who did not have drug. Alcohol drinking, smoking and exercise habits of the elderly who used drug had little effects on the nutrient intakes while those habits had considerable effects on biochemical indices and obesity degree. Among drug users, male drinkers showed higher triglycerides & HDL-cholesterol level, and female drinkers showed higher triglycerides and lower total cholesterol and LDL-cholesterol level, compared to non drinkers. Among female drug users, smokers showed lower body mass index(BMI) compared to non smokers. Males and females who had both drinking and smoking habits showed higher serum triglycerides and VLDL-cholesterol level. Especially females who had both drinking and smoking habits showed lower BMI & Tricep skinfold thickness compared to those who had netiher drinking nor smoking habits.

- To promote health status, strategies and interventions to improve nutrition should be based on the proper diagnosis of the subject's eating patterns. The elderly usually have traditional food habits and preferences, and it is very difficult to change them. This study was designed to identify dietary behavior and food preference of the elderly, in order to provide baseline data for the Elderly Nutrition Intervention Program for the Public Health Center. A survey questionnaire was made for use by trained interviewers to query 151elderly people from 5 community elderly centers located in Suwon, Korea. The majority of them ate regularly and partook of all available side dishes. Their major dietary problems were frequent consumptions of salty foods, and eating too quickly. They consumed grains and vegetables regularly, but seldomly ate dairy products, fruits, meat and food prepared with oil. They also tended to eschew ready made processed food, high cholesterol food, and fast food. Also they did not dine out as much as younger people. Desirable eating habit score were not significantly influenced by socioeconomic variables and nutrition-related characteristics. These included nutrition knowledge, Nutritional Risk Index(NRI) and a score of health concerns. However, meal balance scores were significantly higher in the younger group(p<.05), the higher household income group(p<.05). According to stepwise multiple regression analysis, NRI was the most important determinant of a desirable eating habit score for the male elderly, whereas the score of health concerns was most important for female elderly subjects. The greatest predictor of the meal f balance score was nutrition knowledge. The elderly liked sweet tasting food, grains, rice, stews and Korean style soups. They disliked sour food, dairy products, processed food, and bread. The results indicate that the Elderly Nutrition Education Program should focus on increasing consumption of dairy products, fruits and food with oil, prepared by traditional Korean cooking methods. It also suggests that the program planning should consider the socioeconomic status of the elderly, such as income and education level, as well as concern for health.

- This study examined within- and between-person variation in nutrient intakes in order to estimate the degree of precision in dietary assessment among 37 males and 46 females whose mean age was 70.4 years old. To collect dietary data, each subject was interviewed 5 or 6 times using a 24-hour recall method during a 3 month period. Within-person variation ranged from 23.5% to 101.2%. Lower values of within-person variation were observed in energy, carbohydrated, protein and phosphorous while higher value was observed in vitamin A. Between person variations of nutrient intakes ranged 12.6-23.5% in most nutrients. With 1 day dietary data, observed nutrient intakes were estimated to within 6-25% of the group's usual9true) intakes and 52-198% of the individuals' usual intakes. The values of these maximum percentage deviations became smaller when the number of dietary recalls increased. The results of this study suggest that the Korean elderly subjects appear to have a slightly less diverse diet compared to young Korean women. Within persons, intakes of nutrients largely from animal sources were more variable for the Korean elderly than for their counterparts in Western countries. This study also implies that commonly used 1 day dietary study may be appropriate for assessing group means of nutrient intakes, but clearly not appropriate for assessing individual's nutrient intakes.

- This study was performed to investigate the nutritional status of neurosurgical tube-fed patients. The objective of this study was to improve the nutrition management of tube-fed patients. The current practices of tube feeding and enteral nutrition formula as for 95 patients in the hospital were examined by reviewing patients charts, and interviewing patients, nurses and their family members. The results are summarized as follows ; 57.9% of patients that received the formula showed a decrease in mental status. Among the subjects, 55.7% had nervous system diseases due to old age. Most of the tube-fed patients were hospitalized in the emergency room and while hospitalized, the status of their respiratory organ was abnormal. The method of tube-feeding was by Bolus injection and the type of the tube was a 16Fr size silicone tube. The amount of the injection per day while tube-feeding was on average 1424 kcal for men and 1322 kcal for women and the calories per day ranged from 1000-1500 kcal(50.5%). The injection volume averaged 332.7ml and 45% of patients received more than 300ml per injection. The tube feeding intake rate was 127.9ml/min with 50% of subjects in the range of 50-100ml/min and 10% at 20ml/min. The longer the hospitalization, the older age, coma status, and the higher tube feeding rate, the more decreased were the biochemical parameters.

- This study was done to provide basic information for nutrition education and meal planning by analyzing children's food preference. For this purpose, 938 children from eleven elementary schools under meal service were randomly selected in Taejon. The survey was conducted by questionnaires, and data were analyzed by SAS program. Children prefered ssalbap to japkogbap and liked kimbap the best among one dish foods. But their most favorite foods were chajangmyun in noodles, miyeokkuk in soups, and chamchichigae in pot stew. Sixth graders like kimchichigae more than third graders. In the case of chims, children like duejigalbichim, sogalbichim, and dalgyalchim. In the jorim, they prefered duejigogi jangjorim, mechurial jangjorim to any other jorims. The preference of bockum, namul, jangachi, and jotgal is generally low. Children's preference is high in fruit, drink, fried food, one dish food and is low in jangachi, namul, bockum. These results suggest that the preference of foods should be fully taken into consideration in school meal service so that they can eat foods with pleasure. Foods, which are low in preference due to the cooking method, should be prepared with new cooking methods developed specifically for children. 'Even though the preference of food is low to the children, the nutritious foods should be utilized in the school menu. Then, the imporatance of food intake must be taught to them through nutrition education.

The Recognition Level of Food contamination with Residual Pesticides and Hazardous Heavy Metals in Taejon Area

- This study was to investigate the recognition level of food contamination with residual pesticides and hazardous heavy metals from 365 male and female adults in Taejon area using questionnaires. Among the recognizing level of residual pesticides in overall foods, 69.1% were 'serious', 25.6% were 'average' and 5.3% were 'not serious'. Over 94.7% of the subjects recognized residual pesticides pollution infoods. for hazardous heavy metals in food, 47.8% responsed as 'serious', 40.5% as 'average' and 11.8 as ' not serious' . Over 88.3% of the subjects recognized contaminated pesticides seriously in fruits, 72.1% in vegetables and 51.7% in cereals, whereas 55.7% of the subjects recognized hazardous heavy metal contamination seriously in fruits, 53.4% in vegetables, 40.8% in fishes and shellfishes and 35.0% in seaweeds. The subjects recognized residual pesticides contamination more seriously in overall foods, cereal, potatoes and starches, bean, vegetables and fruits, whereas hazardous heavy metal contamination was recognized more seriously in fishes and shellfishes, and seaweeds food groups. Comparisons were shown based on individuial's occupation. Farmer, forester, iner and fisher showed the lowest recognizing level of food contamination in most food groups. The mean score of the dietary effect by mass media's information on food contamination from residual pesticides and hazardous heavy metals were 3.51+/-0.96 out of 5 points. By Duncan's multiple range test, sex, age, marriage, food cost per month, concerns about health and nutrition knowledge showed significant differences in the mean effect score at p<0.05. According to a pesticides contamination in several food groups were affected by food cost per month, mass media's information on food contamination, health status, and concerns about health, But a recognition level of hazardous heavy metals in food were affected by income and, food cost per month mass media's information on food contamination, health status, concern about health and nutrition knowledge. People who need to take extreme precautions of food contamination were in order of producers, government officials, homemakers, the consumer's association and consumers.
