Risk Factors Affecting the Health of Pregnant Women and Fetus

The aim of this study was to determine the predictors of desirable pregnancy outcomes. The subjects were 795 pregnant women participating in the 2007 Mom and Baby Expo. They were grouped by gestational age: group I (3-12 wk: n = 95), group II (13-25 wks: n = 263) and group III (26-42 wks: n = 437). We collected data for general characteristics, sociocultural factors, life styles and nutrient intakes. We also collected pregnancy outcome data of 634 pregnant women including birth weight, maternal weight gain and gestational age. Dietary intakes of the subjects were estimated by Food Frequency Questionnaire. folate, iron and calcium intakes from foods of pregnant women were 88%, 79% and 58% of KDRIs, respectively. Bivariate analysis showed that birth weight was significantly associated with pre-pregnancy BMI, maternal weight gain, maternal age, gestational age and intakes of iron, potassium, vitaminB1, B6, fatty acids, MUFA. And also, bivariate analysis showed that maternal weight gain was significantly associated with pre-pregnancy BMI, maternal age, gestational age and intakes of energy, potassium. Further multivariate analyses suggest that vitaminB6 may be a significant predictor for low birth weight and energy intake and maternal age for maternal weight gain. Our findings suggest that dietary and lifestyle interventions during pregnancy can improve maternal and infant pregnancy outcomes. Prepregnancy weight control and intakes of energy and vitamin B6 need to be taken into considerations in developing strategic prenatal care programs to promote desirable pregnancy outcome.

A Study on Actual Conditions and Needs of Breastfeeding Education for Pregnant Women in Health Centers

This study was conducted to investigate actual conditions and needs on breastfeeding education for pregnant women in health centers for the development of a breastfeeding educational program. The questionnaires were mailed to 245 health centers and 148 (60.4%) questionnaires were returned. 91% of the health centers had breastfeeding education for pregnant women. 66% of them operated breastfeeding education as one part of other health programs. About 76% of supervisors and 64% of educators were the nurses. The teaching methods frequently used were lectures (30.5%), giving out booklets and leaflets (22.6%), demonstration and practice (21.5%), personal counseling (13.3%), and others. The teaching materials used were materials of outside speakers (39.7%), materials of development oneself (19.0%), materials of academic association or institute (14.8%), and others. The subjects which educators taught were the benefits of breastfeeding (16.7%), breastfeeding techniques (15.8%), caring for breasts before and after delivery (15.1%), nutritional management for lactating women (14.2%), coping strategies for the difficult situation of breastfeeding (13.3%), and others. Those were different from each other according to the educators' general characteristics. Success factors of education were increased motivation for breastfeeding (52.8%), practice (22.6%), professional's lecture (11.3%) and others. The failure factors of education were the ineffectiveness of the lecture method (69.2%), lack of education for supporters (15.4%) and lack of standardized education (15.4%). The most important barrier of education was the lack of a standardized breastfeeding educational programs (43.9%). The most effective teaching methods that educators thought were demonstration and practice (24.0%). The educators thought they need the tools and space for practice (28.2%), a standardized breastfeeding educational program (26.9%), and the human resources (24.4%) for effective education. Subjects that educators thought important for education were the breastfeeding techniques, benefits of breastfeeding, caring for breasts before and after delivery, nutritional management for lactating women, coping strategies for the difficult situation of breastfeeding, rooming system after delivery, ways to assess mother's milk quantity, introducing successful cases of breastfeeding in rank order. To promote the effectiveness of breastfeeding education, standardized breastfeeding educational programs, diverse teaching materials, space and tools, and human resources are needed.

Interrelations Among Fast Food, Beverage Intake and Sociality, Anger Expression of Adolescents in the Busan Area

The purpose of this study was to investigate the relation of the fast food and beverage intake on sociality and anger expression of adolescents. Questionnaires were distributed to the adolescents of 599 middle and high school students in Busan. According to the results, the preference-intake frequency analysis (PEA) on fast food grid, high preference and high intake frequency were 'dukbokki', 'chicken' and 'mandu' and low preference and high intake frequency were 'ramyon', 'gimbab. PEA on beverage grid, high preference and high intake frequency were 'milk dairy product', 'fruit juice', 'isotonic beverage' and low preference and high intake frequency were 'carbonate drink'. The intake frequency of 'pizza', 'sandwich', 'udong', and 'dukbokki' had a positive relationship with sociality. 'Hamburger', 'chicken', 'french fry', 'gimbab', 'mandu', and 'ramyon' showed a positive relationship with anger-out. The intake frequency of 'carbonated drink' had a negative relationship with anger-control, but 'green tea' showed a positive relation with it. 'Carbonate drink', 'isotonic beverage', 'coffee', and 'milkshake' had a negative relationship with anger-out. The explanation power (R2) of intake of fast food and beverage on sociality was 0.019~0.038, and 'carbonated drink' and 'coffee' had a negative influence on sociality. The explanation power (R2) of intake of fast food and beverage on anger expression was 0.011~0.041, and 'carbonated drink' had a negative influence on angercontrol. 'Hamburger', 'carbonated drink', and 'coffee' showed a positive influence on anger-out. From these results, it was necessary to develop the practical eating-out habits program on proper fast food and beverage choice for adolescents.

A Comparative Study of Nutrition Knowledge, Nutrition Attitudes and Eating Behaviors of Dietitians and Non-Dietitians Working in the Metropolitan Area

This study was carried out to compare the nutrition knowledge, nutrition attitudes and eating behaviors of dietitians and non-dietitians. The subjects of this study were 148 dietitians and 195 non-dietitians who are working in the metropolitan area. The general characteristics, nutrition knowledge, nutrition attitudes and eating behaviors of the subjects were surveyed using a self-administered questionnaire. The results were as follows: The result of anthropometric data and body mass index showed no significant differences between dietitians and non-dietitians groups. The dietitian group (16.61 +/- 1.82) showed a significant difference (p < 0.001) compared to the non-dietitian group (14.60 +/- 2.17) in nutrition knowledge scores. Nutrition attitudes of the dietitian group (36.66 +/- 6.91) was a little higher than the non-dietitian group (36.21 +/- 7.95), but not a significant difference. The dietitian group (61.52 +/- 9.37) and the non-dietitian group (62.50 +/- 11.25) were no significant difference in eating behaviors. The correlation between nutrition knowledge scores and nutrition attitudes score of dietitians and non-dietitians groups were no significantly difference and also the correlation between nutrition knowledge scores and eating behaviors scores were not statistically significance. Whereas, the correlation between nutrition attitudes and eating behaviors showed a significant difference (dietitian r = 0.40829, nondietitian r = 0.51914). From these results, we could see the necessity of emphasis of nutrition knowledge by mending the education is necessary to enable dietitians to apply that in real life. Also it is important to guide non-dietitians to adjust their nutrition knowledge, nutrition attitudes and eating behaviors.

Dietary Habits of the Oldest-old Population in Sunchang County

The dietary patterns and characteristics of oldest-old subjects over 85 of Sunchang County (total of 171, 61 males and 110 females) were studied. The average age was 90.6 +/- 3.7 years old (male; 89.1 +/- 2.8, female; 91.5 +/- 3.8), and 90.9% of them were reported to have had no schooling experience. Family types having a spouse were much greater in men (50%) than women (1.8%). The percentages of living alone showed the gender differences, which were comprised of 33% of females and 18.3% of males. In this study, 73.7% of subjects answered "very good" or "good" for their health status. These elderly subjects might maintain their health through a relatively lower rate of smoking (22.2%) and drinking (27.5%), having nonsedentary activities (84.2% of the subjects are physically active) and enough sleep. Their characteristic dietary patterns include high preference of rice (96.5%) and blanch & Seasoned vegetable dishes (Namal, 90.6%) frequent consumption of plant-based food groups containing plenty of anti-oxidants such as vegetables and legumes, and they showed high rates of meal regularity (77.2%) and no skipping of meals (94.2%). It seems to be developed and disseminated in the congregate meal program for improving nutritional status of the elderly, since the coming increment of single-elderly family and single dwellers of rural areas.

The Perception for Management of School Foodservice Using of Environmentally Friendly Agricultural Products of Elementary School Children's Mothers in Gunsan

The purpose of this study was to compare the degree of understanding about school foodservice management and environmentally friendly agricultural products between urban and rural elementary school children's mothers. The subjects were 280 elementary school children's mothers who lived in Gunsan city and the nearby countryside. The results are summarized as follows. The mothers in urban schools were higher on the degree of participation (p < 0.01) and interest (p < 0.01), but were lower on the degree of satisfaction (p < 0.001) for school foodservice than rural ones. The best improvement of school foodservice management was improving food tastes and qualities of the foodstuffs in urban schools and sanitation at the service area in the rural schools (p < 0.001). The school foodservice program contributed to cure the unbalanced diets and developing of bodies and minds; there was no difference of urban and rural schools. But eating habits in rural schools were more improved than urban schools (p < 0.001). The primary reason for using environmentally friendly agricultural products was to improve their health and in securing safe foods, there was no difference of urban and rural schools, but generating the farmer's income from the products in rural schools was higher than urban schools (p < 0.001). There are conflicting views between urban and rural schools for the additional costs brought by using the environmentally friendly agricultural products (p < 0.001). The order of preference on using environmentally friendly agricultural products was rice and various grains, vegetables, fruits, livestock, seasoning, etc. In conclusion, our central and local governments should change their roles in financially positive ways and reflect the issues in making the policy effective. Responsible administrators of school food suppliers run the system more faithfully with the above government support.

Nutrition Management Examined by Plate Waste Measurement: A Comparison with Elementary Schools and Middle Schools in the Changwon Area

The purpose of this study was to examine nutrition management conditions of lunch with the elementary schools and middle schools in the Changwon area. 292 students (5th and 6th graders) from three elementary schools and 330 students from three middle schools (boys, girls, and co-ed) participated in the aggregate selective plate waste measurement for 5 days. Planned menus, serving sizes and plate waste amount data were collected. Nutrient analyses for the planned, served and consumed menus at school lunches were performed by using CAN-PRO 3.0. Nutrient analyses of the planned, served and consumed menus were compared with nutrient management standard (former edition) for school lunch and 1/3 Korean Dietary Reference Intakes (KDRIs). Significant difference was found in the average consumption rate between the elementary schools (82.2%) and the middle schools (71.8%). Specifically, the consumption rates of steamed rice (p < 0.001), side dish 1 (p < 0.001), and Kimchi (p < 0.01) at the middle schools were significantly lower than those of the elementary schools. When the nutrient contents in the served menus were put into percentages to the nutrient contents in the planned menus, middle schools (92.3%) showed bigger serving loss than the elementary schools (95.4%). In the nutrient assessment comparied with nutrient management standard (former edition), middle school lunches showed comparatively less energy or less some nutrient contents against the standard than the elementary school lunches. Specifically, in case of boys in middle schools, Vitamin C was the only nutrient content that satisfied the standard in the planned menus, served menus and consumed menus. In the 1/3 KDRIs based assessment, middle schoolers were found not to be provided proper nutrients with school lunches. To improve nutrition management at middle school foodservices, dietitians should reinforce nutrient assessment for menu planning, and try to decrease serving loss and plate waste.

Developing Job Description for Dietitians Working in Public Health Nutrition Areas

The purpose of this study was to develop a standardized job description for dietitians working in the public health nutrition area. Work-oriented job analysis methodology was employed for the study purpose. Subjects of 38 dietitians currently working at health centers in 2002 were recruited. Based on the focus group interview with 7 public health nutritionists and 7 professors, information about task elements was collected. Questionnaires measuring work performance and self-perception of importance of the selected task elements were administered. Reliability and validity of this instrument were tested by Chronbach's alpha and factor analysis. SAS PC package program was used for the statistical analysis. The final developed job description for public health nutritionists included 5 duties, 20 tasks and 93 task elements. The results of this study can be summarized as follows; 1) 5 duty areas are A. plan and evaluation of public health nutrition services, B. developing nutrition education materials, C. implementing nutrition services, D. networking community, and E. self development. 2) Each duty area from A to E was composed with 6, 2, 6, 4, 2 tasks, respectively. 3) Each duty area from A to E was composed with 24, 8, 38, 14, 9, and 2 task elements, respectively.

A Survey on Blood Pressure and Hematic Parameters During Pregnancy by Women of Premature Delivery

The purpose of this study is to provide basic data for preventing preterm delivery in the aspects of blood pressure and hematic parameters. The blood pressure, hematic parameters, relationship between hematic parameters and nutritional intakes and pregnancy outcomes were compared between a preterm delivery group and a normal term delivery group. The results obtained are summarized as follows. Diastolic blood pressure was statistically higher in the preterm delivery group. White blood cells (p < 0.005) and alanine amino transferase (p < 0.05) of 3rd trimester in pregnancy were statistically higher in the preterm delivery group. Alkaline phosphatase (p < 0.0001) and lactate dehydrogenase (p < 0.05) were statistically lower in the preterm delivery group. Inverse relationships between niacin, vitamin B6 and zinc intakes and bilirubin (p < 0.05) were shown. Vitamin A intakes (p < 0.05) were significantly negatively correlated with blood protein, but zinc intakes (p < 0.05) were significantly positively correlated with blood protein. Vitamin B6 intakes (p < 0.05) were significantly negatively correlated with blood albumin. Calcium intakes (p < 0.005) and iron intakes (p < 0.05) were significantly positively correlated with blood lactate dehydrogenase. Also, vitamin A intakes (p < 0.05) were significantly positively correlated with blood glucose. Normal spontaneous vaginal delivery (p < 0.005) was statistically lower in the preterm delivery group. Birth weight (p < 0.0001) and birth length (p < 0.005) of the neonates were all statistically lower in the preterm delivery group.

The Effect of Collagen Supplementation from Pork Skin on Serum Collagen, Serum Sex Steroid Hormone, Serum Lipid and Skin Crack in Korean Middle-aged Women

This study was performed to examine if the effects of collagen supplementation from pork skin could improve the sex steroid hormone, serum lipid and skin crack in Korean middle-aged women. Middle-aged women (40-55 years) who were not diagnosed with any type of disease were included in this study and thirty subjects were randomly assigned to a control group (n = 15) or a collagen supplemented group (n = 15). The collagen supplemented group ingested collagen flour 2 g, 3 times a day for 12 weeks. We measured serum collagen, estrogen, estradiol, estriol, progesterone, total cholesterol, triglyceride, HDL-cholesterol and LDL-cholesterol concentration. The collagen supplementation group had significantly increased serum collagen (p < 0.05) compared with the control group. In addition, skin crack was improved. But, there were no differences for sex steroid hormone and lipid profile in control and collagen supplemented groups. The result of the present study demonstrated that supplementation of 6 g collagen per day for 12 weeks can give beneficial effects on skin crack reduction and serum collagen concentration.

Effects of Nutrition Education Using Food Exchange System: Changes in Elementary Students' Nutrition Knowledge, Dietary Attitude and Nutrients Intake

The purpose of this study was to investigate the effects of nutrition education using Food Exchange System on elementary students' nutrition knowledge, dietary attitude and nutrients intake. Nutrition education lessons (40 min /lesson, 4 times), '5 major nutrients and functions', '6 food groups and sources of 6 food groups', 'good choice of snacks and eating out' as class lesson, 'daily needed energy and food exchange units' as individual lesson, were provided to 70 elementary students (4th grade 33 students, 5th grade 37 students) in Jeonbuk Province. We assessed the changes in nutrition knowledge, dietary attitude, food habit using a questionnaire and nutrient intake using 24 hr recall method by nutrition education. In nutrition knowledge, there were significant increases in scores of 'functions of carbohydrate', 'functions of protein', 'functions of lipid', 'foods of carbohydrate', and 'foods of lipid'. In dietary attitude, there were significant increases in scores of 'taking a meal with joy', 'taking a meal at ease', 'taking a meal with sufficient protein intake' and 'taking a meal without spicy foods' by nutrition education. There were significant changes in the type of breakfast and in the frequency of snacks. After education, in type of breakfast, it showed higher number of students ate rice oriented meal than they did before education. and in frequency of snacks, it showed lower number of students ate snacks 'over 3 times' than they did before education. In Carbohydrate : Protein : Fat (CPF) ratio (%), it was significantly changed from 55.8 : 17.7 : 26.6 to 63.6 : 15.3 : 21.1. In evaluation of nutrient intake by Dietary Reference Intakes for Koreans (KDRIs), vitamin A, thiamin, niacin, vitamin B6, calcium, phosphate, iron and zinc showed positive changes in distribution of number of children by intake level. That is, there were significant improvements in intakes of vitamin A, thiamin, niacin, vitamin B6, calcium, phosphate, iron and zinc. These results showed that nutrition education using Food Exchange System for elementary students improved nutrition knowledge, dietary attitude and nutrients intake. It suggest that nutrition education using Food Exchange System may improve dietary behaviors and reduce an incidence of obesity in elementary students.
