Process evaluation of a mobile healthcare program among employees with overweight and obesity: a 12-week intervention study investigating the role of engagement

Objectives
A mobile health intervention program was provided for employees with overweight and obesity for 12 weeks, and a process evaluation was completed at the end of the program. We investigated participant engagement based on app usage data, and whether engagement was associated with the degree of satisfaction with the program.
Methods
The program involved the use of a dietary coaching app and a wearable device for monitoring physical activity and body composition. A total of 235 employees participated in the program. App usage data were collected from a mobile platform, and a questionnaire survey on process evaluation and needs assessment was conducted during the post-test.
Results
The engagement level of the participants decreased over time. Participants in their 40s, high school graduates or lower education, and manufacturing workers showed higher engagement than other age groups, college graduates, and office workers, respectively. The overall satisfaction score was 3.6 out of 5. When participants were categorized into three groups according to their engagement level, the upper group was more satisfied than the lower group. A total of 71.5% of participants answered that they wanted to rejoin or recommend the program, and 71.9% answered that the program was helpful in improving their dietary habits. The most helpful components in the program were diet records and a 1:1 chat with the dietary coach from the dietary coaching app. The barriers to improving dietary habits included company dinners, special occasions, lack of time, and eating out. The workplace dietary management programs were recognized as necessary with a need score of 3.9 out of 5.
Conclusions
Participants were generally satisfied with the mobile health intervention program, particularly highly engaged participants. Feedback from a dietary coach was an important factor in increasing satisfaction.

Nutrient Composition and Content of Vitamin and Mineral Supplements and Their Appropriateness for Pregnant and Lactating Women in Korea

OBJECTIVES
Use of dietary supplements containing vitamins and minerals is growing in Korean adults, especially in pregnant and lactating women. Vitamin and mineral supplements are available in different composition and in a wide range of contents. The purposes of the study were to examine nutrient composition and content of vitamin and mineral supplements for pregnant and lactating women and assess their appropriateness as dietary supplements.
METHODS
Information on the name, manufacturer, nutrient composition, and usage of vitamin and mineral supplements for pregnant and lactating women were obtained from the homepage of the Food Safety Information Portal managed by the Ministry of Food and Drug Safety, and Korean Index of Medical Specialties. A total of 264 products were identified.
RESULTS
Among 264 products, 26.1% were single nutrient products, and 73.9% were multinutrient products. The most commonly included nutrient was iron (70.1%), folic acid (66.3%), vitamin B12 (45.8%), vitamin C (38.6%), and vitamin B6 (38.6%). Although more than 50% of products contained nutrients less than 150% of Recommended Nutrient Intakes or Adequate Intakes for daily use, some products contained inappropriately high amounts of nutrients. When a maximum daily dose of supplements was taken as described on the label, iron in 73 products (39.5%), folic acid in 14 products (8.0%) were likely to be consumed in amounts greater than Tolerable Upper Intake Levels. Most products were assessed as inappropriate for pregnant women due to the possibility of excessive intake of vitamins or minerals when compared with Dietary Reference Intakes.
CONCLUSIONS
Pregnant and lactating women need to carefully select dietary supplements containing adequate amounts of vitamins and minerals. Nutritionists should provide guidelines regarding selection of appropriate vitamin and mineral supplements for pregnant and lactating women.

Awareness and Consumption of Energy Drinks and Associated Factors among College Students in Cheongju

OBJECTIVES
This study was conducted to investigate the awareness of energy drinks, energy drink consumption behaviors, and associated factors among college students.
METHODS
A total of 536 students from three universities in Cheongju completed a self-administered questionnaire regarding awareness and consumption of energy drinks as well as general characteristics, health-related characteristics, and eating habits.
RESULTS
Approximately half of the respondents reported they knew what energy drinks were, while 45.7% had heard of energy drinks but did not know what they were. However, 76.9% had experience in using energy drinks. The main reason for using energy drinks was to stay awake, and the main place that they were used was on campus or at the library. More than 70% of energy drink users did not read the caffeine contents of energy drinks. After consuming energy drinks, 31.3% experienced adverse effects, the most serious being sleep disturbance. Approximately 33% of students consumed energy drinks once a month or more frequently, and men consumed energy drinks more frequently than females. Energy drink users who consumed at least one drink each month were more likely to be natural science major or drink more milk for male students, and to have more allowances or consume less meat for female students, compared with non-users.
CONCLUSIONS
Our study showed that 33% of students consumed at least one energy drink each month, and there were a small number of students consumed energy drinks almost every day or energy drinks mixed with alcohol. Accordingly, college students should be taught potential health hazards of energy drinks mixed with alcohol as well as energy drinks alone. In addition, reading labels about caffeine and sugar contents in energy drinks is encouraged.

Development of Education Materials as a Card News Format for Nutrition Management of Pregnant and Lactating Women

OBJECTIVES
The purpose of the study was to develop a series of education materials as a card news format to provide nutrition information for pregnant and lactating women.
METHODS
The materials were developed in seven steps. As a first step, the needs of pregnant and lactating women were assessed by reviewing scientific papers and existing education materials, and by interviewing a focus group. The second step was to construct main categories and the topics of information. In step 3, a draft of the contents in each topic was developed based on the scientific evidence. In step 4, a draft of card news was created by editors and designers by editing the text and embedding images in the card news. In step 5, the text, images and sequences were reviewed to improve readability by the members of the project team and nutrition experts. In step 6, parts of the text or images or the sequences of the card news were revised based on the reviews. In step 7, the card news were finalized and released online to the public.
RESULTS
A series of 26 card news for pregnant and lactating women were developed. The series covered five categories such as nutrition management, healthy food choices, food safety, favorites to avoid, nutrition management in special conditions for pregnant and lactating women. The satisfaction of 7 topics of the card news was evaluated by 140 pregnant women, and more than 70% of the women were satisfied with the materials.
CONCLUSIONS
The card news format materials developed in this study are innovative nutrition education tools, and can be downloaded on the homepage of the Ministry of Food and Drug Safety. Those materials can be easily shared in social media by nutrition educators or by pregnant and lactating women to use.

Development of Nutrition Education Contents for Pregnant Women Based on Effective Communication Strategies

OBJECTIVES
The purpose of the study was to develop communication strategies for effective nutrition education targeting pregnant women and to create nutrition education contents.
METHODS
The format and the contents of online resources on nutrition information for pregnant women provided by reliable institutions were analyzed. Possible solutions to overcome barriers of nutrition education as well as communication strategies for effective nutrition education were identified by a brainstorming process. Based on the communication strategies, contents for nutrition education were created. Understandability, level of interest, applicability to daily life, harmony of text and illustration, and overall satisfaction of the contents were evaluated by dietitians and pregnant women.
RESULTS
The four communication strategies were developed; (1) to focus on a few important messages, (2) to provide evidence-based information, (3) to create illustrations or infographics with a minimum amount of text, and (4) to provide tips on how to improve the current diet options. Based on these strategies, the contents were focused on three important nutrients for pregnant women, folate, iron, and calcium. The percentages of the recommended nutrient intakes of the three nutrients on selected menu and its improved version by adding a dish or changing a dish into another dish were calculated and provided. Finally, the contents were delivered as illustrations with a minimum amount of text. Overall, dietitians and pregnant women were satisfied with the contents.
CONCLUSIONS
The contents developed in this study can be used in a pamphlet or a pregnancy diary, or can be shared in social networking services. Further contents on other nutrients and various menu are expected to be developed using these communication strategies.

Changes in Food and Nutrient Intakes of College Students between 1999 and 2009

The purpose of the study was to examine the changes of food and nutrient intakes of college students between 1999 and 2009. Dietary survey of 169 college students was conducted by a 24-hour recall method for three days in 2009. Food and nutrient intakes in 2009 were compared with the data from 106 students collected by the same methods in 1999. The intakes of cereals & grain products and vegetables in 2009 were lower than those of 1999, but the intakes of meats, eggs, milk & milk products, and manufactured food were higher. The intake of rice per person decreased greatly from 452.2 g in 1999 to 351.4 g in 2009 in males, and from 306.9 g to 237.2 g in females. While the intakes of protein, fat, thiamin, niacin, vitamin B6, phosphorus, potassium, zinc, and cholesterol were significantly higher, the intakes of dietary fiber were significantly lower in 2009 compared to 1999 both in males and females. The nutrients consumed less than the Recommended Intakes were vitamin A, riboflavin, vitamin C, and calcium in males and additionally folate, iron, and zinc in females in both 1999 and 2009. The ratio of carbohydrate, protein and fat as energy was 61 : 15 : 24 and 60 : 14 : 26 in 1999, and 54 : 16 : 30 and 56 : 15 : 29 in 2009 in males and females respectively, showing that carbohydrate intake decreased and fat intake increased greatly. Our data suggest that nutrition education is necessary for college students to help them consume more vegetables and fruits and less fat and cholesterol.

Effect of Nutrition Education Program Developed by a Public Health Center on Preschool Children's Nutrition Knowledge and Dietary Habits and the Parent's Dietary Attitudes

This study was carried out to examine the effects of nutrition education program developed by Jincheon Public Health Center on preschool children's nutrition knowledge and dietary habits and the parents' dietary attitudes. The subjects of this study were five- and six-year-old children as well as their parents. A 5-week nutrition education program was implemented to 104 children in five day care centers, and 107 children in three day care centers were investigated as a control group. Activity tools designed for each lesson such as puzzles, food magnets, story, songs, Pierrot costume, and balls were used. Nutrition knowledge and dietary habits of children and dietary attitudes of parents were evaluated before and after education. Mean nutrition knowledge score in the education group was significantly higher than that in the control group after education (p < 0.001). Mean dietary habit score of three questions (three meals a day, eating at fixed time, eating breakfast) in the education group was also significantly higher than that in the control group after education (p < 0.01). In addition, parents in the education group showed significantly higher mean dietary attitude score than those in the control group even though they did not receive education (p < 0.001). Our nutrition education program was found to be effective in improving nutrition knowledge and dietary habits in preschool children as well as improving dietary attitudes in their parents.

Dietary Habits, Food Frequency and Dietary Attitudes by Gender and Nutrition Knowledge Level in Upper-grade School Children

This study was carried out to examine dietary habits, food frequency and dietary attitudes by gender and nutrition knowledge level in upper-grade school children. Subjects were 223 boys and 208 girls attending 13 elementary schools (5 th and 6 th graders). A self-administered questionnaire was developed to assess nutrition knowledge, dietary habits, food frequency, and dietary attitudes. Nutrition knowledge score assessed by 19 questions was significantly higher in girls than in boys. There were significant differences between boys and girls in several items in dietary habits such as enough breakfast time, dinner regularity, number of side dishes, eating speed, type of snack after dinner, and degree of saltiness of dishes. Girls consumed staple foods such as rice, noodles or bread more frequently, and consumed less frequently ramyun, fried foods, fast foods, and Chinese foods than boys. In addition, girls had more positive dietary attitudes in trying to reduce fast foods, Chinese foods, ramyun and fried foods. Also children in the high or moderate nutrition knowledge level groups had better dietary habits in frequency of meals, frequency of breakfast, enough breakfast time, dinner regularity, activity during the meal, number of side dishes, eating speed, type of snack after dinner, and better food choices by eating staple foods and vegetable side dishes, meats and beans more frequently, and fried foods, doughnuts/ cakes, Chinese foods less frequently, and had better dietary attitudes in breakfast, vegetables and fruit intake than children in the low nutrition knowledge level group. Our results showed that dietary habits, food frequency and dietary attitudes were different by gender and nutrition knowledge level. These results suggest that in developing nutrition education program for elementary school children, the differences between gender and nutrition knowledge level should be considered. Nutrition education in elementary schools may change dietary habits and dietary attitudes through increasing nutrition knowledge.

Effect of Prune Supplementation on Dietary Fiber Intake and Constipation Relief

The purpose of this study was to examine bowel habits, dietary habits, and nutrient intake of constipated adults, and the effects of prune products on relieving constipation symptoms. Fifty one adults with self-reported constipation (mean age 23 years, range 19-41 years, 10 males and 41 females) participated in this study. After a baseline survey on bowel habits and dietary habits, participants were asked to consume at least 50 g of prune and 200 ml of prune juice per day during a 4-week period in addition to usual diet. Nutrient intake was estimated by a 24 hour recall at the baseline and once every week by diet records during the intervention. Data were analyzed after classifying the subjects into mild constipation group and severe constipation group by the severity of the symptoms. During the intervention, the subjects with mild constipation consumed 56 g of prunes (about 5.6 fruits) and 200 ml of prune juice, and the subjects with severe constipation consumed 59 g of prunes (about 5.9 fruits) and 207 ml of prune juice. Average intakes of energy, dietary fiber and water of the subjects in the mild constipation and severe constipation group increased during the intervention compared to the baseline. Average dietary fiber intake of the mild constipation and severe constipation groups significantly increased from 12.5 g and 11.6 g at the baseline to 18.5 g and 16.8 g after consuming prune products, respectively. These changes were accompanied by an increase in the number of bowel movements, a decrease of defecation time, a change to a softer stool consistency, and a decrease of abdominal pain during defecation. Seventy two subjects answered that prune products were effective to improve their overall constipation symptoms. Our data show that supplementation of prune products is effective to provide energy, dietary fiber and water, and to relieve constipation symptoms for constipated adults.

Amounts and Food Sources of Nutrients of Elementary School Lunch Menus by the Type of Foodservice and the Percent Energy from Fat

This study was conducted to assess macro- and micronutrient compositions in school lunch menus based on the 'Dietary Guideline' for School Lunches. Ninety-five dieticians in elementary school in Chungbuk were asked to complete a questionnaire on characteristics of the school lunch program (such as type of foodservice, food production system), the information about dietitians (such as age, education, and job history), and the extent of the use of processed foods and frozen foods. They were asked to return the questionnaire with the menus including the name and the quantity of every food ingredient offered during a week. A total of 554 lunch menus provided for a week of June 2004 were analyzed. Average nutrient content per meal was as follows; 660 kcal, energy; 92.9 g, carbohydrate; 26.7 g, protein; 21.1 g, fat; 287 microgramRE, vitamin A; 0.5 mg, thiamin; 0.5 mg, riboflavin; 29.3 mg, vitamin C; 338.2 mg, calcium; 3.9 mg, iron; and 97 mg, cholesterol. Average percentages of energy from carbohydrate, protein and fat was 56.2%, 16.2%, and 29.0%, respectively. The mean nutrient content per meal was higher in rural-type than in urban-type schools. The weekly menu of 40% of the schools provided < 55% of energy from carbohydrate, and 39% of the schools offered lunch that provided > or =30% of energy from fat. The micronutrient content was generally high when the percent energy from fat was less than 25%. Our results showed that only 52.6% of the schools provided lunches with the energy composition as in the 'Dietary Guideline' of School Lunches. Whole Milk was the major contributor to fat, saturated fatty acid and cholesterol. We suggest that school foodservices start to provide low fat milk instead of whole milk to reduce fat, saturated fatty acid and cholesterol. If low fat milk is served instead of whole milk, percentage of energy from fat and saturated fat can be reduced from 29% to 25%, and from 10.2% to 9.1%, respectively, and cholesterol could be reduced from 97 mg to 79 mg. Efforts to meet 'Dietary Guideline' for School Lunches should be made, especially to reduce fat intake, while maintaining essential nutrient intake at sufficient levels for children.

Blood Folate Level Determined by a Microplate Reader and Folate Intake Measured by a Weighted Food Record

Microbiological method using a 96-well microplate reader for folate assay was established, and folate intake and blood folate concentrations of 23 female college students were assessed. To evaluate folate intake, dietary data were collected by a 3-day weight food record, and serum and RBC folate concentrations were measured by the new method. The coefficient of variation for the new method was less than 10%. Mean daily folate intake of the subjects was 126.7ug which is only 50.7% of the RDA. The mean concentrations of serum and RBC folate were 7.46ng/ml and 294.4ng/ml, respectively, which were within the normal range. These results indicate that folate intake seems to be underestimated due to incomplete food composition database. Therefore, folate database should be appropriately in order to asses folate intake accurately.
