Changes in nutritional status of Korean older adults during COVID-19 Pandemic by household income and demographic factors -using the Korea National Health and Nutrition Examination Survey(2019-2020): a cross-sectional study

Objectives
The study aim was to identify changes in the nutritional status of older adults during the COVID-19 pandemic according to household income and demographic characteristics.
Methods
Study participants were 2,408 adults aged 65 and over who participated in the 2019–2020 Korea National Health and Nutrition Examination Survey (KNHANES). To examine changes in nutrient intake levels resulting from COVID-19, data of 2019 and of 2020 were compared. Study participants were divided into three groups based on household income level to compare these changes. The changes were compared according to household income level, age group, and household type.
Results
Percentages of recommended intakes for energy, protein, and most micronutrients were the lowest for the low-income group of both males and females in 2020. The Mean Adequacy Ratio (MAR) score was the lowest for the low-income group in both years. When comparing nutrient density for 2019 and 2020 by income group, the male low-income group experienced a decrease in nutrient densities of vitamin A, thiamine, calcium, and iron. For the same group, a decreased percentage for energy intake from protein was noted. Fruit intake was lowest in the low-income group for both males and females. Low-income males had the lowest intake levels for meat, fish, eggs, and legumes in both 2019 and 2020 and the lowest milk and milk product intake levels in 2020. Older adults living alone or single older adults with children had lower MAR scores than those living with a spouse. Older adults living alone experienced decreases in energy and thiamine and iron intake levels in 2020 compared to their intake levels in 2019.
Conclusions
Because of the COVID-19 pandemic, nutrition intake levels worsened for older adult males in the low-income group and older adults living alone. This finding shows the need for a more systematic nutritional support strategy for the vulnerable older adults population in national disaster situations.

Biochemical Characteristics and Dietary Intake according to Household Income Levels of Korean Adolescents: Using Data from the 6th (2013 ~ 2015) Korea National Health and Nutrition Examination Survey

Objectives
The purpose of this study was to examine the biochemical characteristics, intake of energy, and nutrients by household income levels of Korean adolescents aged 12 to 18 years.
Methods
Data from the 6th (2013 ~ 2015) Korea National Health and Nutrition Examination Survey (KNHNES) were used for the study. A total of 1,839 (966 boys, 873 girls) subjects were included, and they were divided into four income groups according to their household income level. We examined general characteristics (gender, region of residence, skipping or not-skipping breakfast, lunch, dinner, frequency of eating-out), anthropometric characteristics (height, weight, weight status), biochemical characteristics (fasting plasma glucose, blood urea nitrogen, creatinine, triglycerides, cholesterol, HDLcholesterol, hemoglobin, and hematocrit), the quantitative intake of energy and nutrients using the Korean Dietary Reference Intakes (KDRI), and the qualitative intake evaluated by the nutrition adequacy ratio (NAR) and mean nutrition adequacy ratio (MAR) of the four groups.
Results
There were significant differences by income group within the region of residence and the rate of skipping breakfast, lunch, and dinner. The low-income group had a higher rate of skipping breakfast, lunch, and dinner. According to the income group, there was a difference in the height of boys, and there was no difference in the weight and obesity of boys and girls. In the biochemical characteristics, only the hematocrit of girls showed differences by income group. The quantitative intake of energy and nutrients compared with KDRI differed by income group. There were differences in energy, carbohydrates, proteins, thiamin, riboflavin, niacin, and phosphorus levels in boys and protein, vitamin A, niacin, and sodium levels in girls. The qualitative intake of energy and nutrients examined using NAR and MAR also differed according to the income group. The NAR showed differences in calcium in boys and vitamin C and calcium in girls. The MAR revealed differences in both boys and girls by income group.
Conclusions
Among adolescents in the low-income group, the rate of skipping meals was high, and the quantitative and qualitative intake of energy and some nutrients was low. It is suggested that the nutritional intake can be improved by lowering the rate of skipping breakfast, lunch, dinner. We suggest that even just providing breakfast in schools can be considered highly effective in improving the rate of avoidance of skipping meals and improving nutrient intake. Also, we suggest that it is necessary to improve the food environment, food availability, and food accessibility through national and social support for low-household income adolescents.

Qualitative Study on the Perception of Community Food-accessibility Environment among Urban Older Adults

Objectives
This study explored the community food environmental factors affecting food purchasing using a qualitative research methodology for the elderly as well as the various food environments under their socioeconomic diversity.
Methods
For the qualitative data collection, this study interviewed 20 elderly people aged 65 years or more, who participated in a public health program or lunch services operated by the senior welfare center in Seoul. Five dimensions, such as availability, physical accessibility, affordability, acceptability, and accommodation suggested in previous studies, were used to identify the community food environmental factors.
Results
The elderly participants showed overall similarities to the concepts derived from existing studies on the five dimensions of food accessibility environment. In addition, other important food accessibility environmental factors that were not present in previous studies, such as acceptability for a product of domestic origin, delivery service to home, and small-packaged food sales, were derived. On the other hand, the concept of some subjects differed depending on the household income and specifically for the physical accessibility concept. This showed that the close distance factor from a grocery store at home might not apply to older adults in low-income households in Korea.
Conclusions
This study found that five dimensions of the food environment suggested by previous studies could also be applied to vulnerable older adults in Korea. On the other hand, the socioeconomic characteristics of individuals and households would affect the perspectives of their local food environments differently. The findings of this study could help in the development of tools for evaluating the community food environment.

Shifts in Kimchi Consumption between 2005 and 2015 by Region and Income Level in the Korean Population: Korea National Health and Nutrition Examination Survey (2005, 2015)

OBJECTIVES
The importance of kimchi as a traditional food in Korean cuisine has gradually decreased due to rapid industrialization, economic growth and changes in dietary patterns in Korea. This study aimed to examine the shifts in kimchi consumption by region and by income level between 2005 and 2015 in Korea.
METHODS
Data from the Korea National Health and Nutrition Examination Surveys III (2005) and VI (2015) were used in the study (n=15,558). Intake of kimchi was estimated using a single 24-hour dietary recall. The sample weights were applied in all analyses to reflect population estimates. All statistical analyses were carried out by using SPSS IBM Statistics 20.
RESULTS
Kimchi intake has significantly decreased by 27.6 g/day per capita during the last decade in Korea; 25.0 g for males and 29.9 g for females, respectively. Over the past decade, the decline of kimchi intake has been particularly significant in Seoul, Busan, Incheon, Gyeonggi, and Gwangju, while there has been no significant change in males living in rural areas. The consumption of kimchi across all income levels has decreased, however, the decrease was higher in ‘middle and low income level’. The amount of kimchi consumption in 2015 was the highest in ‘low income level’. The results were similar after adjusting for gender and age.
CONCLUSIONS
For the past decade, the overall intake of kimchi in Korea has decreased, however, it has been found that the decrease of kimchi intake for males living in the rural areas was not significant. Therefore, in order to keep our traditional kimchi culture and promote a balanced diet including kimchi for Korean, it is necessary to develop more efficient policies and approaches. A variety of dishes using kimchi should be developed, besides merely serving kimchi with rice as a side dish, to increase the consumption of kimchi.

The Development of a Nutrition Education Program for Low-income Family Children by applying the Social Cognitive Theory and Health Belief Model

OBJECTIVES
Based on individual and environmental characteristics of low-income children, we developed a nutrition education program for school-aged children from low-income families according to effective use in social welfare centers.
METHODS
We conducted in-depth group interviews to assess program needs in 28 participants, 10 low-income school-aged children and 9 of their care givers, 9 social workers and 9 care-givers. Theoretical backgrounds of our program were heath belief model and social cognitive theory considering motivation, action and environment characteristics.
RESULTS
Based on the findings of this qualitative study, we developed major program themes and contents. Five selected key themes were 'balanced diet', 'processed food', 'food hygiene and safety', 'Korean healthy traditional diet', and 'family cooking' to induce changes in dietary behaviors. Main findings of in-depth group interviews included 'child's active participation', 'simple and easy to understand messages', and 'environmental constraints' such as a lack of child care at home, limited budget of social welfare centers, and less qualified educators for nutrition and health. Each lesson was constructed as a 1-hour program particularly emphasizing activity-based programs, including cooking and teamwork exercises. Program contents in each session consisted of activities that could induce outcome and value expectations, self-efficacy, perceived benefits, and barriers and cues to actions regarding diet behavior.
CONCLUSIONS
We developed a nutrition education programthat is rarely available for low-income children in Korea, considering theoretical bases. Further studies are needed to validate our program.

A Qualitative Study on Attitude, Acceptability, and Adaptation for Home-delivered Meal Services in the Korean Elderly from the Perspective of Life Context

OBJECTIVES
The purpose of this study was to suggest the strategies for improvement of home-delivered meal services for the elderly, to identify reasons for recipients to get started with the services and to evaluate the attitude, acceptability and adaptation of recipients to the services from the perspective of life context.
METHODS
The data was collected through face-to-face in-depth interviews with eighteen low-income elderly recipients of home-delivered meals and analyzed using a qualitative research method.
RESULTS
The results were deduced as four themes which comprised of long-term vulnerable socioeconomic contexts resulted in entry to the services, conflicting acceptability to the services, passive adaptation to taking the services, and positive practices to cope with supplement free meals or other services. The service participation was initiated because of a combination of prolonged, vulnerable socioeconomic contexts, including poverty and unexpected life events such as diseases, disability, living alone, aging and unemployment. With regard to taking the services, conflicting acceptability was observed: positive aspects including saving living cost and good quality of meals, and negative aspects including lack of a tailored service and feeling of stigma. Although the recipients needed an individualized service, they did not express their needs and demands for the services and they accepted the unavailability as an accustomed, prolonged vulnerable socioeconomic context. With regard to lack of tailored services, either self-solution such as modification of eating patterns or community-based network and services were used.
CONCLUSIONS
We suggest that a system to concretely identify recipients' attitude, acceptability and adaptation for home-delivered meal services should be developed in the establishment of a tailored nutrition support system for the low-income elderly.

A Study on the Health and Nutritional Characteristics according to Household Income and Obesity in Korean Adults Aged over 50: Based on 2005 KNHANES

This study investigated the health and nutritional characteristics according to household income level and obesity in Koreans aged over 50 years based on the 2005 National Health and Nutrition Examination Survey. Subjects were classified into 3 groups by average household income with reference to the minimum monthly living expenses (MLE): low (n = 319, < 100% MLE), middle (n = 222, < 200% MLE), high (n = 411, > or = 200% MLE) and each group was compared by BMI index. With increasing income level, the prevalence of systolic hypertension and hyperlipidemia was increased. In the low income group, serum total cholesterol, triglycerides, and fasting glucose were higher in the obese compared with the normal. In the middle and high income groups, the prevalence of hyperlipidemia and diastolic hypertension were higher in the obese. Subjects had nutritional imbalance, such as inadequate intake of calcium and potassium. With increasing income level, the percentages of protein and fat to total calorie were increased in addition to the intakes and density of nutrients. The obese in the low income group had higher intakes of energy, protein, phosphorus and higher consumption frequency of cereals and potatoes compared with the normal. It was shown that the obese of the middle and high income groups tended to have lower consumption frequency of Korean cabbage and higher frequency of fruits. The obese of high income group also had binge drinking habit. Therefore, this study suggests that specific approaches based on economic status should be considered in developing nutrition education program for the elderly.

A Study on Customized Nutrition Intervention Program Design and Application for the Low-Income Elderly

This study aimed to plan nutrition support programs for the elderly living alone whose nutrition status were seriously concerned, conducted seven stages nutrition intervention program on a trial basis, and evaluated the effectiveness of the program of the Elderly Nutrition Support Project. Subjects were selected for personalized nutrition management based on nutritional risk score and nutrition intervention were tailored to the problems occurred. The elderly nutrition support program targets were 44 senior citizens who lived alone with low income. The 33 (as Type 1) of the subjects with whom milk, tofu, seaweed, eggs, black beans have been supported, and also provide nutrition education, and the rest 11 persons (as Type 2) to whom food was not supported but provide nutrition education programs. As a result, all subjects showed that compared with pre and post program implementation, their daily exercise time and milk and protein consumption level were increased and some improvement was observed regular meals consumption and low-salt diets. Their nutrient intake level such as calories, protein, calcium, iron improved after implementation. In addition, NSL DETERMINE scores significantly improved from 13.21 to 7.24 in Type 1 and 11.27 to 9.91 in Type 2. As positive dietary behavioral changes were observed as in that they purchased more protein and calcium rich foods.

Diet Quality and Food Patterns of Obese Adult Women from Low Income Classes: Based on 2005 KNHANES

This study aims to identify the dietary patterns relevant to obesity of Korean women among low income classes. Adults 20-64 years were used as study subjects from the data of 2005 Korea National Health and Nutrition Examination Survey. We compared obese and normal-weight women in terms of their nutrients intake, diet quality and food patterns. Diet quality was assessed by using the Nutritional Adequacy Ratio (NAR) and Index of Nutritional Quality (INQ). Our results showed higher prevalence of obesity among lower socioeconomic status women. In men, there were no significant associations with socioeconomic status and prevalence of obesity. Higher risk of nutritional inadequacy was observed among obese women compared to normal weight women. Obese women showed significantly lower INQ for nutrients such as Ca, Fe, Vitamin A, Thiamin, Riboflavin and Vitamin C compared to other women. They consumed significantly higher amount of rice (p < 0.05) and lower amount of vegetables (p < 0.01). By contrast, obese men from low income classes showed higher intake of those nutrients. Obese men also consumed significantly higher amount of meats than normal weight men. Therefore, this study suggests that genderspecific approaches based on economic situation should be considered in developing the intervention program for managing obesity for low income classes.

Government-Funded Meal Support Program for Low-Income Children through Convenience Stores : Current Status and Nutritional Quality of Available Meal Items in Seoul

The objectives of this study were to investigate the current status of the Korean government-funded meal support program for low-income children through convenience stores and to evaluate the nutritional quality of the meal items available under the program. The POS data of three convenient stores where children had used their electronic meal cards most often in Seoul during January 2010 and the kinds and amounts of ingredients of the meals items available to the children were obtained from the headquarter of the convenient stores. A total of 5,081 transactions by 693 children included in the POS data was analyzed. In addition, nutritional contents of meal items, which were meal boxes (11 kinds), kimbab (13 kinds), rice balls (27 kinds), inari sushi (1 kind), and sandwiches (26 kinds), were analyzed with Can Pro 3.0. The results showed that children had purchased flavored-milk products most often. Children tended to purchase meal items together with drinks (60.9% of transactions), but some purchased drinks (27.6%) or meal items only (11.5%). Except for meal boxes, none of the meal items satisfied 1/3 of Estimated Energy Requirements of the 9-11 year-old boys per day. The average energy contents of different kinds of meal boxes, kimbabs, rice balls, and sandwiches were 619, 357, 200, and 380 kcal, respectively, and the energy content of a package of Inari sushi was 457 kcal. Vitamin C amount was found to be deficient in all the meal items, compared to 1/3 of Recommended Intake of the 9-11 year-old boys per day. The results of this study could be useful to develop nutritionally appropriate meal items for the convenient stores participating in the government-funded meal support program for children from low-income families.

Appropriate Size and Dish Combination of Nutritional-Balanced Lunch Boxes Delivered to Children Under the Government-Funded Meal Service Program in Korea

The purpose of this study was to propose the appropriate sizes and dish combination for nutritional-balanced lunch boxes delivered to children under the government-funded meal service program in Korea. The study was based on the 3 : 1 : 2 Meal Box Magic, a nutrition education method developed in Japan. A total of 290 lunch menus, comprising of 10 day menus from 29 organizations having delivered lunch boxes to children during summer vacation of 2008, were analyzed and used as the base data for lunch box combination. Dishes of the menus were classified into 6 groups: Rice group, Protein side dish group (including meat, fish, egg, and bean dishes), Vegetable side dish group, Kimchi group (including kimchi and jangajji), Soup, stew group, and the other group. Nutrient analysis was conducted for 100 ml of these dishes by CAN Pro 3.0 utilizing volume and weight conversion data used for analysis of the Korea Health and Nutrition Examination Survey and those from the Nutrient Composition of Food for Consumers. All the possible combinations of 5 dishes, comprising of 3 dishes from Rice group, Protein side dish group, and Kimchi group, respectively, and 2 dishes from Vegetable side dish group, were made using the frequently served dishes from the respective dish group. Nutrient analysis of each combination was conducted based on the assumption that a lunch box was 600 ml and filled up to 80% by dishes using the volume ratio of 3 : 1 : 1.5 : 0.5 for Rice group : Protein side dish group : Vegetable side dish group : Kimchi group. The mean and standard deviation of energy and nutrients of all combinations calculated by weighting the serving frequency of each dish selected for the combinations were 621 +/- 81 kcal for Energy, 22.1 +/- 5.0 g for Protein, 120 +/- 45 mg for Calcium, 4.1 +/- 1.1 mg for Iron, 201 +/- 130microgram RE for Vitamin A, 0.34 +/- 0.10 mg for Thiamin, 0.27 +/- 0.10 mg for Riboflavin, and 24.3 +/- 9.6 mg for Vitamin C. The energy percentages from Carbohydrate, Protein and Fat were 66%, 14% and 20%. The analysis results met the nutrition standard of lunch boxes for male elementary students in grades 4 through 6 under the government-funded meal service program regarding calories, nutrients except calcium and riboflavin, and macronutrient distribution ranges. Accordingly appropriate box sizes were suggested for different age and sex groups to meet the respective nutrition standards. In addition, milk or dairy products were suggested to accompany lunch boxes to supplement calcium and riboflavin intake. The method of selecting box sizes and making dish combination suggested in this study could be useful for the organizations preparing lunch boxes under the government-funded children's meal service program where nutrition professionals are not available.

Current Status of Meal Box Service Management for Children from Low-income Families During Summer Vacation

The purpose of this study was to investigate the current status of foodservice management in organizations delivering meal boxes for low-income children during summer vacation. A survey was conducted with persons in charge of meal box production and service of these organizations via mail. Out of 114 questionnaires distributed nationwide, 100 were analyzed (87.8% analysis rate). Over half (53%) of the organizations delivered meal boxes consisting of rice and side dishes while the rest delivered side dishes only. About 81% of the organizations received KRW 3,000 per meal from their local governments and the rest received KRW 3,500. Only 28% of organizations had employed a dietitian. Over one-third (38%) of the respondents were unaware of the official nutritional standard of the foodservice program for low-income children during vacation. Most of the organizations (94%) had menu planned in advance. The average percentage of food cost was 84.1%. Over 40% of the organizations did not keep food samples for sanitation test (43%) and did not take any measures for keeping food temperature during delivery (45%). The organizations delivering rice and side dishes were more likely to be located in cities rather than rural areas and received higher reimbursement rate. The organizations receiving reimbursement of KRW 3,500 or hiring a dietitian were more likely to use standardized recipes, keep food samples for sanitation test, or take measures for keeping food temperature during delivery compared to the counterparts. Respondents reported that increasing reimbursement rate was the most necessary for improving the quality of meal box. This study results showed that the meal box delivery service for low-income children was not properly managed during the vacation, with regards to menu planning and food production. It is recommended that reimbursement rate for meal boxes should be adjusted depending on meal box types and local conditions.

Nutrient Intakes Differences of the People Living Near the Nuclear Plant by the Household Income Level

This study was conducted to measure and evaluate the food and nutrient intakes of the people living near the nuclear plant and to investigate the relationship between the household income level and the food and nutrient intake patterns. A total of 552 cases (263 males and 289 females) were surveyed during the period from April 1 to December 21 of 2005. Dietary intake was measured by means of the 24-hour recall method. The data were analyzed using SPSS Windows (ver. 14.0). The household income level of the subjects was classified into two groups : Low income group (LIG; < or = 2,000,000 won) and high income group (HIG; > 2,000,000). The subjects at large had less energy and nutrient intakes than did the population in town and village who participated in the 2005 National Health and Nutrition Survey. The intake of calcium, zinc, vitamin A, riboflavin, vitamin B6, vitamin C, and folic acid was less than the Estimated Average Requirement in case of 50~95% of the subjects. The LIG consumed less beans, vegetables, fruits, meats, and beverages than did the HIG in male, while the LIG consumed less eggs and beverages than did the HIG in female. The LIG consumed less nutrients than did the HIG in male, except for carbohydrate, while the LIG consumed less nutrients including zinc, vitamin A, riboflavin, vitamin B6, vitamin C, folic acid than did the HIG in female. In addition, the LIG had higher percentage energy consumption from carbohydrate. These results suggest that higher food and nutrient intake is associated with higher income.

A Comparative Study on the Dietary Attitudes and Nutritional Status of Preschool Children in Different Income Levels in Busan

This study was carried out to investigate the dietary attitudes and nutritional status of preschool children in different income levels in Busan. The survey was conducted from November 1 to November 30, 2004 by questionnaires and data analyzed by the SPSS program. The results are summarized as follows: The Kaup index of the upper income class group was higher than that of the other income classes. In the middle income class group, nutrition knowledge scores were higher than in any other income class. Sixty-four point nine percent of the upper income class children, 25.7% of the middle income class children and 15.7% of the low income class children had nutrition education experiences. Intake of protein was higher than that of the Korean recommended dietary allowances (RDAs) for children. In the upper income class group, intake of calcium was higher than in any other income class. Intakes of calcium and iron were lower than those of the RDAs in all children. Dietary attitude had positive correlation with frequency of breakfast, time for outdoor activity, and height and weight, and had negative correlation with frequency of fast food and time for watching television. Frequency of snack had positive correlation with frequency of fast food and time for watching television, but frequency of breakfast had negative correlation with those. These results indicate that nutrition education programs linking parents to children should be carried out to improve their nutritional status.

Physical Development and Dietary Behaviors of Children in Low-Income Families of Seoul Area

Since the economic crisis in 1997, the number of low income families has increased and the turmoil is likely to affect nutritional and health status of the children in low-income families. The purpose of this study was to investigate dietary behaviors and physical development of low income family children in the Seoul area. The subjects were 44 boys and 56 girls, between the ages of 7 and 12 years, from the beneficiary families of the governmental assistant program and enrolled in after school-care centers. A self-reported questionnaire, including general characteristics and dietary behaviors, was applied to the children and their parents. Skipping meal rate was higher in breakfast than in lunch or dinner, and was less frequent in parents and children families compared to that of the other types of families (p<0.05). Most students are shown that they usually keep the dietary guideline for Korean children. There were significant differences in keeping the dietary guidelines between the parents and children family group and other groups in several items. The results imply that the nutrition education program should be more focused on the families which are not composed of parents and children.

To explore the relationship between economic status and food and nutrient intake patterns, the 2001 National Health and Nutrition Survey result was analyzed. Dietary intake data of 6,978 Korean adults of 20 years and older who participated in the 2001 National Health and Nutrition Survey were used along with their demographic data. Economic status of the subjects was classified into the following 4 groups based on the self-reported average monthly income of household with reference to the minimum monthly living expenses (MLE) in 2001: low<100% MLE < or = middle<200% MLE < or = high<300% MLE < or = higher. Individuals in the higher income class had significantly higher mean intake for most of the nutrients including energy, protein, carbohydrate, fat, calcium, thiamin, riboflavin, niacin and vitamin C, and a higher percentage of energy intake from fat. In addition, they consumed more animal foods including meats, eggs, fish/shellfish, milk/dairy products and fats. On the other hand, the mean intakes of individuals in the lower economic class for calcium, vitamin A, and riboflavin were lower than 75% of RDAs. And, there was a predominant difference in contribution of fat to total energy intake among the groups of different economic status. These results showed that household income is an important factor influencing the food and nutrient intake patterns of the Korean adult population. Although individuals at different age classes may respond differently to a change in economic status, developing and implementing nutrition policy and intervention programs for those nutritionally vulnerable groups should consider the economic status as an important factor to customize and differentiate the content of the program.

Gender and Age Differences in the Nutritional Status of the Low Income Elderly Living in Gwangju

This study was undertaken to assess nutritional status of the low income elderly residing in Gwangju. Anthropometric data showed that mean height of the subjects was lower than that of Korean Standard Growth data but weight was similar to that standard value. As the subjects became older, their heights and weights were decreased. Body mass index (BMI) in males and females were 22.9 and 24.4 respectively. However, BMI distribution showed that 56% of the elderly females under 75 were underweight. Advancing age conoibutes significant difference in triceps skinfold thickness in females (p<0.001). Total cholesterol level was higher in the elderly females than the males. It was found that a considerable number of subjects had anemia determined by hemoglobin and hematocrit level, which indicated un iron deficiency. Twenty-four hour dietary recall revealed that, except for phosphorus and vitamin C, all the nutrient intakes of the subjects were below 75% of Korean RDA. Surprisingly, vitamin A and riboflavin intakes of the elderly were below 50% of Korean RDA. Energy intakes of the elderly males and females were 58.7% and 59.6% of Korean RDA respectively. Unbalanced energy ratios of carbohydrate, protein and fat were noted in both genders. Nutrient intakes of females' were lower than those of males'. Thus, there was a significant gender difference in nutrient intakes. It might be further suggested that an appropriate nutritional program should be developed and implemented to improve the Poor nutritional status of the low income elderly living in cities.

Nutritional Intake Status of the Elderly Taking Free Congregate Lunch Meals Compared to the Middle-Income Class Elderly

Comparative studies regarding the nutritional status of 93 home-living elderly people taking free congregate lunch meals(FL) and 87 middle income class elderly people(MI) were performed in Taejon city. Data was obtained from questionaires, anthropometry and interviews for the 24-hour dietary recall of 2 nonconsecutive days during August, 1996. The average age for FL was 75.8 years. The monthly familly income for FL belonged to the low-income class. FL females had lower heights and weights than MI females. The average daily nutritional intake of both FL and MI were low, particularly in FL whose %RDA of energy was 68.5%, protein 65%, Ca 29.6%, Fe 50.8%, vitamin A 34.5%, vitamin E 30.5%, riboflavin 40.6%, vitamin C 76.9%. MI's %RDA of energy was 76.4%, protein 80.a2%, Ca 48.1%, Fe 78.6%, vitamin A 67.3%, vitamin E 117.4%, riboflavin 45.6%, vitamin C 136.5%. Comsumption of Zn, vitamin B6 and folic daily average. There was no nutrient having average INQ(Index of nutritional quality) over 1 for either group. The INQs for protein, Ca, Fe and vitamin A were 0.802, 0.377, 0.625 and 0.296 in FL, and 0.900, 0.601, 0.784 and 0.602 in MI, respectively. The MAR(Mean adequacy ratio) was low with the value of 0.500-0.518 in FL and 0.630-0.723 in MI. The percentage of main nutrients from lunch was the highest among the three meals for FL males, while that from breakfast was the highest for MI. Free lunches taken by FL supported higher percentages of main nutrients than home-lunches taken by MI. Eating-out was done more frequently by MI than by FL and that eating-out brought them more nutritional intake. The above data indicated that the dietary nutritional intake status of the FL elderly was very poor in both quantity and quality and that free congregate lunch significantly contributed to the daily nutrient-intake for the FL elderly.

The purpose of this study was to evaluate the nutritional status and growth of Korean infants, who were atending peripheral community clinics in low income areas, by anthropometric measurements and estimating dietary intakes. Dietary intakes and growth were compared among different feeding patterns of 143 infants until age 9 months. The overall mean nutrient intakes of infants in this study were below the recommended allowances except the calcium intake from significant difference in each groups; however, calcium, zinc and iron intake of the formula fed infant(FF) was higher than the breast fed infant(BF) or the mixed fed infant(MF). Form ages 4-6 months, the nutrient intakes were shown to be higher in groups that were given supplementary foods than groups that were not. From ages 7-9 months, all nutrient intakes were higher in or the formula and supplementary foods fed(ESF) infants than in the breast and supplementary food(BSF) or the formula and supplementary food(FSF) groups, All subjets in this study showed a large Z-score. The growth of infants up to 6 months of age showed no significant difference in the feeding pattern, however, after 7 months of age the BSF group had significantly lower weight than the FSF or the ESF groups, There were significant positive relationships between infants weight gain at age 7-9 months from birth and the current protein or zinc intakes. As a result the average status of nutrient intakes of infants in this area was loser than the RDA, however, the growth pattern was fairly good. Although the breast milk is beneficial for infants, mothers should be educated for the importance of supplemental food and its practice to support good nutrition.

Korea has not tried any food consumption survey so far except the national nutrition survey, which does not show food consumption patterns of different income stratas. The results of the family income and expenditure survey(FIES) by the national statistical office can be precious sources which show household food consumption patterns due to large, random. Samples, year-round survey period and socioeconomic background data. This study analyzed the FIES data to find out food consumption patterns including nutrient intakes and frequently consumed foods by households among different monthly income levels. Big difference was found in food consumption patterns among the quartile-income groups especially the amount of consumed foods, food expenditure, and nutrient intakes. For every food item, the higher the monthly invomr, yhr motr og goof yhry vondumrf. The monthly food expenditure of higher higher income strata was composed with higher percentage of relatively expensive foods compared to other stratas. Nutrient intake levels of lower income strata were 50-60% of the RDA, which showed the necessicity of food assistance programs for those high risk groups to complement the nutritional difficiency.

This study was aimed at evaluating the nutritional status of Korean lactating women, who were attending peripheral community clinics in low-income areas, by anthropometric measurement, estimating dietary intakes and analyzing the contents of breast milk components, serum lipid content and fatty acid composition. Regarding anghropometric measurements, maternal circumferences decreased significantly at the trunk but not at the limbs. The skinfold thickness on all regions had the tendency to decrease during lactation. It was found that intakes of energy, protein, lipid and carbohydrates were desirable while calcium, zinc, copper and vitamin A were low and sodium was excessive. The energy % of fat was 22.41% of the total energy which is higher than the RDA for adults but it is proper for lactating women, but energy % from each fatty acid was usually low and an intake ratio of omega6/omega3 fatty acids was desirable. Accordingly, it is thought that dietary fat intake was balanced in quality. Concentrations of the nutrients in matured human milk showed the same range as that reported form different regions with the exception of calcium and magnesium which were relatively low. Serum lipid contents were decreased with the time postpartum and the average serum lipid content of lactating women were similar to the normal range of adults. Serum polyunsaturated fatty acids. especially omega6 fatty acids have gradually increased with the time postpartum, but the composition were not greatly influenced by dietary intakes.
