Process evaluation of a mobile healthcare program among employees with overweight and obesity: a 12-week intervention study investigating the role of engagement

Objectives
A mobile health intervention program was provided for employees with overweight and obesity for 12 weeks, and a process evaluation was completed at the end of the program. We investigated participant engagement based on app usage data, and whether engagement was associated with the degree of satisfaction with the program.
Methods
The program involved the use of a dietary coaching app and a wearable device for monitoring physical activity and body composition. A total of 235 employees participated in the program. App usage data were collected from a mobile platform, and a questionnaire survey on process evaluation and needs assessment was conducted during the post-test.
Results
The engagement level of the participants decreased over time. Participants in their 40s, high school graduates or lower education, and manufacturing workers showed higher engagement than other age groups, college graduates, and office workers, respectively. The overall satisfaction score was 3.6 out of 5. When participants were categorized into three groups according to their engagement level, the upper group was more satisfied than the lower group. A total of 71.5% of participants answered that they wanted to rejoin or recommend the program, and 71.9% answered that the program was helpful in improving their dietary habits. The most helpful components in the program were diet records and a 1:1 chat with the dietary coach from the dietary coaching app. The barriers to improving dietary habits included company dinners, special occasions, lack of time, and eating out. The workplace dietary management programs were recognized as necessary with a need score of 3.9 out of 5.
Conclusions
Participants were generally satisfied with the mobile health intervention program, particularly highly engaged participants. Feedback from a dietary coach was an important factor in increasing satisfaction.

Facilitators and barriers to achieving dietary and physical activity goals : focus group interviews with city bus drivers and counseling dietitians

Objectives
Our previously published study showed that a workplace nutrition intervention program with personalized goal setting and smartphone-based nutrition counseling improved dietary habits and physical activity in city bus drivers who were overweight/obese. This study explored the facilitators and barriers that participants faced in achieving their dietary and physical activity goals six months after the intervention.
Methods
The study included bus drivers and dietitians who participated in the intervention program. Three focus group interviews were conducted with 10 bus drivers (divided by two groups based on their achievement of set goals) and five dietitians who had provided nutrition counseling.
Results
Willpower was the most important intrapersonal facilitator for drivers to achieve their goals. Other factors that promoted behavioral changes were nutrition counseling by dietitians, trackable physical activity using smartwatches, and setting of practical and achievable goals. Meanwhile, the most important barriers identified were occupational factors such as long driving hours, short breaks, and shift work. Other barriers were environmental factors such as availability of snackable food, accessibility to convenience stores, and cold weather. Family and colleagues were perceived as both facilitators and barriers. In addition, dietitians identified a lack of knowledge about healthy diet as one of the barriers.
Conclusions
Our results suggested that the workplace environment should be improved and that nutrition intervention programs at the workplace could encourage bus drivers to practice healthy eating habits. The facilitators and barriers identified in this study should be considered when planning a nutrition intervention program for bus drivers.

Screen time, mealtime media use, and dietary behaviors in Korean preschoolers : a cross-sectional study

Objectives
Screen time refers to the time spent using screen media, such as televisions, smartphones, computers, or tablets. Excessive exposure to screen media has been reported to negatively impact young children’s health and development, including overweight, short sleep duration, and language delays. This study examined the association of screen time and mealtime media use with dietary behaviors among preschool children.
Methods
A cross-sectional survey was conducted on parents of children aged three to five years using the online questionnaires of the Nutrition Quotient for Preschoolers (NQ-P) and the Dietary Screening Test (DST). Data from 261 children’s parents were analyzed.
Results
Of the 261 children, 96.9% used screen media, 55.6% used screen media for two hours or more daily, and 30.7% were exposed to screen media during meals. The NQ-P scores were significantly lower in the children with longer screen time and mealtime media use. Children who used screen media for two hours or more and those exposed to screen media during meals consumed kimchi less frequently and confectionery and sugar-sweetened beverages more frequently than children who used less than two hours and were not exposed to screen media during meals. In addition, they were more likely to be picky about food, refuse to eat, and less likely to feed themselves than children with shorter screen time and no mealtime media use.
Conclusions
This study reported an association between unhealthy dietary behaviors, feeding difficulties, and screen time and mealtime media use among preschool children. Further research should explore effective strategies for reducing children’s screen time.

Effectiveness of a mobile health intervention on weight loss and dietary behavior changes among employees with overweight and obesity: a 12-week intervention study investigating the role of engagement

Objectives
This study aimed to determine whether a mobile health (mhealth) intervention is effective in reducing weight and changing dietary behavior among employees with overweight and obesity. The study also investigated whether engagement with the intervention affected its effectiveness.
Methods
The intervention involved the use of a dietary coaching app, a wearable device for monitoring physical activity and body composition, and a messenger app for communicating with participants and an intervention manager. A total of 235 employees were recruited for a 12-week intervention from eight workplaces in Korea. Questionnaire surveys, anthropometric measurements, and 24-h dietary recalls were conducted at baseline and after the intervention.
Results
After the intervention, significant decreases in the mean body weight, body mass index, body fat percentage, and waist circumference were observed. Furthermore, the consumption frequencies of multigrain rice and legumes significantly increased, whereas those of pork belly, instant noodles, processed meat, carbonated beverages, and fast food significantly decreased compared with those at baseline. The mean dietary intake of energy and most nutrients also decreased after the intervention. When the participants were categorized into three groups according to their engagement level, significant differences in anthropometric data, dietary behaviors, and energy intake were observed following the intervention, although there were no differences at baseline, indicating that higher engagement level led to greater improvements in weight loss and dietary behavior.
Conclusions
The intervention had positive effects on weight loss and dietary behavior changes, particularly among employees with higher engagement levels. These results indicate the importance of increasing the level of engagement in the intervention to enhance its effectiveness. The mhealth intervention is a promising model for health promotion for busy workers with limited time.

Nutritional Status of Intensive Care Unit Patients According to the Referral to the Nutrition Support Team and Compliance with the Recommendations

Objectives
This study aimed to examine the effectiveness of the intervention of the nutrition support team (NST) on the nutritional status of critically ill patients.
Methods
The medical records of 176 adult patients who were admitted to the intensive care unit and received enteral or parenteral nutrition for more than 7 days were retrospectively analyzed. The patients were classified into the NST and non-NST groups according to whether they were referred to the NST or not. The NST group was further classified into the compliance and non-compliance groups depending on their compliance with the NST recommendations.
Results
The NST referral rate was 56.8%, and the rate of compliance with the NST recommendations was 47.0%. Significantly higher energy and protein were provided to the NST and the compliance groups than to the non-NST and the non-compliance groups. The proportion of patients who reached the target calories after the initiation of enteral nutrition was significantly higher in the NST and the compliance groups than in the non-NST and the non-compliance groups. The serum albumin and hemoglobin levels significantly decreased in every group, but the changes were significantly lower in the compliance group. The nutritional status at discharge from the intensive care unit compared to the status at admission was significantly worse in the NST, non-NST, and non-compliance groups. However, the status was maintained in the compliance group. The length of stay in the intensive care unit was significantly shorter in the compliance group.
Conclusions
Compliance with the NST recommendations was found to provide more calories and protein and prevent the deterioration of the nutritional status of critically ill patients. Therefore, effective communication between medical staff and the NST from the early stages of admission to the intensive care unit is needed to improve referrals to the NST and compliance with the recommendations.

Interpersonal and Community Factors Related to Food Sufficiency and Variety: Analysis of Data from the 2017 Community Health Survey

Objectives
This study examined the personal, interpersonal and community factors related to food sufficiency and variety among Korean adults using data from the 2017 Community Health Survey. Methods A total of 228,310 adults aged ≥ 19 years were classified into three groups: food sufficiency with variety, food sufficiency without variety and food insufficiency. Personal factors included sociodemographic characteristics, health behavior and health status. Interpersonal factors included social networking and social activities, and community factors included safety, natural environment, living environment, availability of public transportation and health care services. The association of food sufficiency and variety with interpersonal and community factors was assessed using multivariable logistic regression analyses. Results Of the total sample, the food-sufficiency-without-variety group and food insufficiency group accounted for 31.5% and 3.2%, respectively. The sociodemographic factors associated with food insufficiency and non-variety were women, ≥ 65 years of age, with low education level, low household income, unemployed, single, and living in areas of small population sizes. There were significant differences in health behavior and health status, interpersonal and community factors among the three groups. Multivariable logistic regression analyses conducted after adjusting for confounding factors showed that lack of social networking and social activities and lower satisfaction derived from community environments were associated with the risk of food insufficiency and non-variety. Conclusions Our results showed that interpersonal and community factors as well as personal factors were related to food sufficiency and variety. Therefore, public policies to help build social networks and participation in social activities, and improve community environment are needed together with food assistance to overcome the problems of food insufficiency and non-variety.

Evaluation of Nutritional Content in Convenience Store Lunchboxes by Meal Type, Price, and Store Brand

Objective
This study investigated the menu and nutritional contents of convenience store lunchboxes, and evaluated the nutritional content by meal type, price, and store brand.
Methods
In September 2019, 93 convenience store lunchboxes from the top five franchise stores were purchased. Relevant information on price, food weight, food ingredients, cooking methods, and nutrition labeling were subsequently collected. Nutritional content was evaluated based on the daily value (DV) and Index of Nutritional Quality (INQ), and energy contribution of carbohydrate, protein, fat, saturated fat and sugar was compared with the recommended range.
Results
Most lunchboxes included the food groups; grains/starches, meats/fish/eggs/ legumes, and vegetables. However, none provided fruits, and only a few lunchboxes provided milk/milk products. Stir-frying, deep-frying, and pan-frying were the most frequent methods of cooking. The average energy content of the lunchboxes was 736.6 kcal, whereas the average contents of protein, fat and saturated fat were higher than 40% of the DV, and sodium content was 66.8% of the DV. The contents of most nutrients in traditional type lunchboxes were higher, as compared to nutrients in onedish type lunchboxes. Considering pricing of lunchboxes, protein and sodium contents were higher in the higher-priced lunchboxes as compared to lower-priced lunchboxes, but there were no differences in the INQs. The contents of energy, protein, fat, saturated fat, and cholesterol significantly differed by brand.
Conclusions
Our results indicate that convenience store lunchboxes contain high levels of protein, fat, saturated fat, and sodium. The nutritional contents differed by meal type, price, and store brand, and higher price did not imply higher nutritional quality. We propose the need to educate consumers to check nutrition labels and purchase appropriate lunchboxes. Manufacturers also need to make efforts to reduce the amounts of fat, saturated fat, and sodium, and not provide protein in excess.

Analysis of Dietary Calcium and Phosphorus Intakes and Contribution Rates of Major Dish Groups according to Gender, Age, and Region in Korea

OBJECTIVES
Calcium (Ca) is an insufficiently consumed nutrient, whereas phosphorus (P) intake has exceeded the recommended intake level in Korea over the past decade. The purpose of this study was to analyze dietary Ca and P intakes and their contribution rate according to dish groups.
METHODS
A 24-hour dietary recall survey of 640 healthy adults (aged 19–69 years) was undertaken twice in four Korean provinces. Dietary Ca and P intakes and their rates of contribution from 31 major dish groups were analyzed and compared by gender, age group, and region.
RESULTS
The average Ca and P intakes of the subjects were 542.1 ± 222.2 mg/d and 1,068.3 ± 329.0 mg/d, respectively. The intakes of Ca and P as percentages of recommended nutrients intake (RNI%) were 71.7 ± 29.8% and 152.6 ± 47%, respectively, and the percentages under the estimated average requirement were 60.3% for Ca and 3.8% for P. The RNI% of Ca was not significantly different between males and females, but was significantly higher in subjects in the sixties age group than in other age groups and was significantly lower in the Korean capital than in other regions. The RNI% of P did not significantly differ by gender or age groups, but it was significantly higher in the capital than in Gyeong-sang. The five major dish groups contributing to Ca intake (contribution rate) were milks/dairy products 69.2 ± 109.2 mg/d (12.6%), soups 55.6 ± 69.6 mg/d (10.1%), stir-fried foods 53.1 ± 70.7 mg/d (9.7%), stews 43.4 ± 85.4 mg/d (7.9%), and kimchi 38.4 ± 31.8 mg/d (7.0%). The five major dish group contributing to P intake (contribution rate) were cooked rice 160.7 ± 107.1 mg/d (14.9%), stir-fried foods 88.5 ± 89.4 mg/d (8.2%), soups 76.7 ± 85.8 mg/d (7.1%), one-dish meals 63.3 ± 94.4 mg/d (5.9%), and stews 62.6 ± 89.3 mg/d (5.8%). The dish groups contributing to Ca and P intakes differed somewhat by gender, age group, and region.
CONCLUSIONS
Programs to improve the nutritional status of Ca and P intakes should consider the differences in Ca and P contribution rates by dish groups as well as by gender, age group, and region.

Nutrient Composition and Content of Vitamin and Mineral Supplements and Their Appropriateness for Pregnant and Lactating Women in Korea

OBJECTIVES
Use of dietary supplements containing vitamins and minerals is growing in Korean adults, especially in pregnant and lactating women. Vitamin and mineral supplements are available in different composition and in a wide range of contents. The purposes of the study were to examine nutrient composition and content of vitamin and mineral supplements for pregnant and lactating women and assess their appropriateness as dietary supplements.
METHODS
Information on the name, manufacturer, nutrient composition, and usage of vitamin and mineral supplements for pregnant and lactating women were obtained from the homepage of the Food Safety Information Portal managed by the Ministry of Food and Drug Safety, and Korean Index of Medical Specialties. A total of 264 products were identified.
RESULTS
Among 264 products, 26.1% were single nutrient products, and 73.9% were multinutrient products. The most commonly included nutrient was iron (70.1%), folic acid (66.3%), vitamin B12 (45.8%), vitamin C (38.6%), and vitamin B6 (38.6%). Although more than 50% of products contained nutrients less than 150% of Recommended Nutrient Intakes or Adequate Intakes for daily use, some products contained inappropriately high amounts of nutrients. When a maximum daily dose of supplements was taken as described on the label, iron in 73 products (39.5%), folic acid in 14 products (8.0%) were likely to be consumed in amounts greater than Tolerable Upper Intake Levels. Most products were assessed as inappropriate for pregnant women due to the possibility of excessive intake of vitamins or minerals when compared with Dietary Reference Intakes.
CONCLUSIONS
Pregnant and lactating women need to carefully select dietary supplements containing adequate amounts of vitamins and minerals. Nutritionists should provide guidelines regarding selection of appropriate vitamin and mineral supplements for pregnant and lactating women.

Awareness and Consumption of Energy Drinks and Associated Factors among College Students in Cheongju

OBJECTIVES
This study was conducted to investigate the awareness of energy drinks, energy drink consumption behaviors, and associated factors among college students.
METHODS
A total of 536 students from three universities in Cheongju completed a self-administered questionnaire regarding awareness and consumption of energy drinks as well as general characteristics, health-related characteristics, and eating habits.
RESULTS
Approximately half of the respondents reported they knew what energy drinks were, while 45.7% had heard of energy drinks but did not know what they were. However, 76.9% had experience in using energy drinks. The main reason for using energy drinks was to stay awake, and the main place that they were used was on campus or at the library. More than 70% of energy drink users did not read the caffeine contents of energy drinks. After consuming energy drinks, 31.3% experienced adverse effects, the most serious being sleep disturbance. Approximately 33% of students consumed energy drinks once a month or more frequently, and men consumed energy drinks more frequently than females. Energy drink users who consumed at least one drink each month were more likely to be natural science major or drink more milk for male students, and to have more allowances or consume less meat for female students, compared with non-users.
CONCLUSIONS
Our study showed that 33% of students consumed at least one energy drink each month, and there were a small number of students consumed energy drinks almost every day or energy drinks mixed with alcohol. Accordingly, college students should be taught potential health hazards of energy drinks mixed with alcohol as well as energy drinks alone. In addition, reading labels about caffeine and sugar contents in energy drinks is encouraged.

Development of Education Materials as a Card News Format for Nutrition Management of Pregnant and Lactating Women

OBJECTIVES
The purpose of the study was to develop a series of education materials as a card news format to provide nutrition information for pregnant and lactating women.
METHODS
The materials were developed in seven steps. As a first step, the needs of pregnant and lactating women were assessed by reviewing scientific papers and existing education materials, and by interviewing a focus group. The second step was to construct main categories and the topics of information. In step 3, a draft of the contents in each topic was developed based on the scientific evidence. In step 4, a draft of card news was created by editors and designers by editing the text and embedding images in the card news. In step 5, the text, images and sequences were reviewed to improve readability by the members of the project team and nutrition experts. In step 6, parts of the text or images or the sequences of the card news were revised based on the reviews. In step 7, the card news were finalized and released online to the public.
RESULTS
A series of 26 card news for pregnant and lactating women were developed. The series covered five categories such as nutrition management, healthy food choices, food safety, favorites to avoid, nutrition management in special conditions for pregnant and lactating women. The satisfaction of 7 topics of the card news was evaluated by 140 pregnant women, and more than 70% of the women were satisfied with the materials.
CONCLUSIONS
The card news format materials developed in this study are innovative nutrition education tools, and can be downloaded on the homepage of the Ministry of Food and Drug Safety. Those materials can be easily shared in social media by nutrition educators or by pregnant and lactating women to use.

Development of Nutrition Education Contents for Pregnant Women Based on Effective Communication Strategies

OBJECTIVES
The purpose of the study was to develop communication strategies for effective nutrition education targeting pregnant women and to create nutrition education contents.
METHODS
The format and the contents of online resources on nutrition information for pregnant women provided by reliable institutions were analyzed. Possible solutions to overcome barriers of nutrition education as well as communication strategies for effective nutrition education were identified by a brainstorming process. Based on the communication strategies, contents for nutrition education were created. Understandability, level of interest, applicability to daily life, harmony of text and illustration, and overall satisfaction of the contents were evaluated by dietitians and pregnant women.
RESULTS
The four communication strategies were developed; (1) to focus on a few important messages, (2) to provide evidence-based information, (3) to create illustrations or infographics with a minimum amount of text, and (4) to provide tips on how to improve the current diet options. Based on these strategies, the contents were focused on three important nutrients for pregnant women, folate, iron, and calcium. The percentages of the recommended nutrient intakes of the three nutrients on selected menu and its improved version by adding a dish or changing a dish into another dish were calculated and provided. Finally, the contents were delivered as illustrations with a minimum amount of text. Overall, dietitians and pregnant women were satisfied with the contents.
CONCLUSIONS
The contents developed in this study can be used in a pamphlet or a pregnancy diary, or can be shared in social networking services. Further contents on other nutrients and various menu are expected to be developed using these communication strategies.

OBJECTIVES
The purpose of the study was to develop a dietary fiber composition table (DFCT) and to assess dietary fiber intakes in Korea National Health and Nutrition Examination Survey (KNHANES).
METHODS
The DFCT was developed by compiling the food composition tables published by the agencies of Korea, United States, or Japan. When there was no available data from the same species or status (dried, boiled, etc.) of food, the values were imputed by estimating from the same species with different status or substituting familiar species in biosystematic grouping. Using KNHANES VI-2 (2014) microdata and DFCT, intake of dietary fiber of Koreans was estimated.
RESULTS
Among the 5,126 food items of DFCT, the proportion of items of which dietary fiber contents were taken from the analytical values of the same foods was 40.9%. The data from the domestic food composition tables was 37.5%, and the data from the foreign tables was 49.6%. The rest was assumed as zero, or estimated with recipe database and nutrition labeling. Mean daily intake of dietary fiber was 23.2 g, and mean intake per 1,000 kcal was 10.7 g in men and 12.6 g in women. The mean percentage of dietary fiber intake compared to adequate intake was higher than 100%. The major food groups contributing to dietary fiber intakes were vegetables and cereals, and the percent contribution were 32.9% and 23.0% of total dietary fiber intakes, respectively.
CONCLUSIONS
This DFCT could serve as a useful database for assessing dietary fiber intakes and for investigating the association between dietary fiber intakes and non-communicable diseases.

Weight Control Behaviors, Health-related Quality of Life and Nutritional Status by Overestimation of Body Image among Young Korean Females: Data from the Fifth Korea National Health and Nutrition Examination Survey, 2010-2011

OBJECTIVES
This study was performed to investigate the effects of overestimation of their own body shape on weight control behaviors, mental condition, physical activity, dietary behavior, health-related quality of life, and nutritional status among young Korean females.
METHODS
A total of 1,514 women aged 20-39 years who are not pregnant and lactating among those who participated in the Fifth Korea National Health and Nutrition Examination Survey (2010~2011), were analyzed and grouped into underestimation (n=120), normal estimation (NE, n=840), and overestimation (OE, n=554), based on their body perception compared to actual percent ideal body weights. Variables were compared between OE group and NE group.
RESULTS
The subjects in OE group were significantly younger (29.5 vs. 30.5 years, respectively p < 0.05) and had lower body mass index (20.9 vs. 22.2 respectively, p < 0.01), health-related quality of life measured by EQ-5D (EuroQol-5 Dimension)(0.97 vs. 0.98 respectively, p < 0.05), nutrition adequacy ratio (NAR) of protein (0.91 vs. 0.93 respectively, p < 0.05), phosphorous (0.93 vs. 0.96 respectively, p < 0.05), and iron (0.70 vs. 0.75 respectively, p < 0.01), and index of nutrition quality (INQ) of iron (0.84 vs. 0.89 respectively, p < 0.05) compared to those in the NE group. The results of logistic regression showed that unmarried status (OR 1.32; 1.01~1.72) and severely underweight/underweight status (OR 1.94; 1.01~3.75)/(OR 1.81; 1.34~2.45) were significantly related to the probability of overestimation by the subjects. Significantly more women in OE group practiced fasting, skipping meals, and taking prescribed pills to lose body weights and had skipped breakfast compared to those in the NE group.
CONCLUSIONS
The results of this study suggested that the Korean young women's distorted perception of own body shape may be associated with undesirable weight control behavior, low quality of life, and lower nutritional status. Therefore, nutrition education for this group should include information on correct body shape perception and its importance.

Development and Evaluation of a Nutrition Education Website for the Prevention and Management of Childhood Obesity

Childhood obesity has rapidly increased worldwide and is one of the most serious health problems in this age group. In order to prevent and manage childhood obesity, we developed a nutrition education website. The website consisted of three parts. The first part was made for self-assessment with regard to obesity index, dietary habits, food frequency, dietary attitude, nutrition knowledge, nutrient intake, energy expenditure, and the stage of behavioral change, and tailored messages and advice according to the assessment results. A total of 612 real-size food photos as well as a nutrient database of 3,346 foods and 541 dishes were created to help children estimate nutrient intakes accurately. In addition, an energy expenditure database of 156 activities for children was established to calculate calorie consumption. The second part was made for setting long-term and short-term goals and keeping track of the changes in energy intake and expenditure in one's own page. The third part was made for education. Various types of nutrition information were provided; texts, pictures, calculators and games. The readability and design of the website were evaluated by 46 obese children. Usefulness, design and readability of the website were found to be desirable for children. This website is expected to be used by an obese child alone or with parents or nutrition teachers in order to control body weight through healthy dietary habits and physical activities. In addition, a non-obese child can also use this website for maintaining healthy dietary habits and preventing obesity.

Changes in Food and Nutrient Intakes of College Students between 1999 and 2009

The purpose of the study was to examine the changes of food and nutrient intakes of college students between 1999 and 2009. Dietary survey of 169 college students was conducted by a 24-hour recall method for three days in 2009. Food and nutrient intakes in 2009 were compared with the data from 106 students collected by the same methods in 1999. The intakes of cereals & grain products and vegetables in 2009 were lower than those of 1999, but the intakes of meats, eggs, milk & milk products, and manufactured food were higher. The intake of rice per person decreased greatly from 452.2 g in 1999 to 351.4 g in 2009 in males, and from 306.9 g to 237.2 g in females. While the intakes of protein, fat, thiamin, niacin, vitamin B6, phosphorus, potassium, zinc, and cholesterol were significantly higher, the intakes of dietary fiber were significantly lower in 2009 compared to 1999 both in males and females. The nutrients consumed less than the Recommended Intakes were vitamin A, riboflavin, vitamin C, and calcium in males and additionally folate, iron, and zinc in females in both 1999 and 2009. The ratio of carbohydrate, protein and fat as energy was 61 : 15 : 24 and 60 : 14 : 26 in 1999, and 54 : 16 : 30 and 56 : 15 : 29 in 2009 in males and females respectively, showing that carbohydrate intake decreased and fat intake increased greatly. Our data suggest that nutrition education is necessary for college students to help them consume more vegetables and fruits and less fat and cholesterol.

Effect of Nutrition Education Program Developed by a Public Health Center on Preschool Children's Nutrition Knowledge and Dietary Habits and the Parent's Dietary Attitudes

This study was carried out to examine the effects of nutrition education program developed by Jincheon Public Health Center on preschool children's nutrition knowledge and dietary habits and the parents' dietary attitudes. The subjects of this study were five- and six-year-old children as well as their parents. A 5-week nutrition education program was implemented to 104 children in five day care centers, and 107 children in three day care centers were investigated as a control group. Activity tools designed for each lesson such as puzzles, food magnets, story, songs, Pierrot costume, and balls were used. Nutrition knowledge and dietary habits of children and dietary attitudes of parents were evaluated before and after education. Mean nutrition knowledge score in the education group was significantly higher than that in the control group after education (p < 0.001). Mean dietary habit score of three questions (three meals a day, eating at fixed time, eating breakfast) in the education group was also significantly higher than that in the control group after education (p < 0.01). In addition, parents in the education group showed significantly higher mean dietary attitude score than those in the control group even though they did not receive education (p < 0.001). Our nutrition education program was found to be effective in improving nutrition knowledge and dietary habits in preschool children as well as improving dietary attitudes in their parents.

Amounts and Food Sources of Nutrients of Elementary School Lunch Menus by the Type of Foodservice and the Percent Energy from Fat

This study was conducted to assess macro- and micronutrient compositions in school lunch menus based on the 'Dietary Guideline' for School Lunches. Ninety-five dieticians in elementary school in Chungbuk were asked to complete a questionnaire on characteristics of the school lunch program (such as type of foodservice, food production system), the information about dietitians (such as age, education, and job history), and the extent of the use of processed foods and frozen foods. They were asked to return the questionnaire with the menus including the name and the quantity of every food ingredient offered during a week. A total of 554 lunch menus provided for a week of June 2004 were analyzed. Average nutrient content per meal was as follows; 660 kcal, energy; 92.9 g, carbohydrate; 26.7 g, protein; 21.1 g, fat; 287 microgramRE, vitamin A; 0.5 mg, thiamin; 0.5 mg, riboflavin; 29.3 mg, vitamin C; 338.2 mg, calcium; 3.9 mg, iron; and 97 mg, cholesterol. Average percentages of energy from carbohydrate, protein and fat was 56.2%, 16.2%, and 29.0%, respectively. The mean nutrient content per meal was higher in rural-type than in urban-type schools. The weekly menu of 40% of the schools provided < 55% of energy from carbohydrate, and 39% of the schools offered lunch that provided > or =30% of energy from fat. The micronutrient content was generally high when the percent energy from fat was less than 25%. Our results showed that only 52.6% of the schools provided lunches with the energy composition as in the 'Dietary Guideline' of School Lunches. Whole Milk was the major contributor to fat, saturated fatty acid and cholesterol. We suggest that school foodservices start to provide low fat milk instead of whole milk to reduce fat, saturated fatty acid and cholesterol. If low fat milk is served instead of whole milk, percentage of energy from fat and saturated fat can be reduced from 29% to 25%, and from 10.2% to 9.1%, respectively, and cholesterol could be reduced from 97 mg to 79 mg. Efforts to meet 'Dietary Guideline' for School Lunches should be made, especially to reduce fat intake, while maintaining essential nutrient intake at sufficient levels for children.

Evaluation of a Nutrition Education Website for Children

This study was carried out to evaluate the usefulness and the effectiveness of the nutrition education website 'ifood (http://ifood.or.kr)' for children. A total of 262 children (5th and 6th graders) participated in the self-education program using the website which consisted of 4 sessions every two weeks. In each session, they were asked to answer some questions on the comprehensiveness of each topic while navigating the website. Nutrition knowledge (20 items), dietary attitudes (7 items), and dietary behaviors (10 items) were evaluated using questionnaires before and after the education. After the education, the usefulness and readability of the website was also evaluated. Most (97.2%) of the responders reported that the education using this website was helpful, 97.3% thought that the contents were reliable, 90.1% responded that the contents were easy to understand, and 88.8% answered that they would revisit 'ifood'. The menus they liked best were "Interesting games" for 35.8%, "How can I cook?" for 17.8%, "How can I become slim" for 16.4%. This website might be more attractive by providing a "greater variety of contents in detail", "more music, motion pictures and images", "more interesting games". After the education, there was a significant increase in nutrition knowledge scores from 10.9 to 13.1. Although the total scores of dietary attitudes and dietary behaviors did not improve, there was a significant increase in each of some questions. These results showed that "ifood", a nutritional education website, was desirable in reliability, aesthetics and enjoyability, and was effective in improving nutrition knowledge and some of dietary attitudes and behaviors in children.

Differences in Characteristics and Dietary Habits between Volunteers and Selected Subjects in Nutrition Survey

This study was designed to compare general characteristics and dietary habits between volunteers for a nutrition survey and non-volunteers using a questionnaire. Volunteers were recruited by advertising on the homepage of a university to assess nutritional status by examining dietary intake for three days and blood analysis. Non-volunteers were selected from some classes not related to nutrition. There were no significant differences in sex, monthly allowances, eating-out cost, drinking and exercise between the two groups, while the proportion of self-boarding was higher in the volunteer group than in the non-volunteer group, and smoking rate of volunteers was approximately 2.4 times lower than that of non-volunteers. Volunteers were less concerned about their diet than non-volunteers. No significant differences in concern about health, considering factors in their diet, self-evaluation of their diet, learning experience about nutrition, and vitamin/mineral supplement use were observed. The frequency and the place of eating-out for dinner were significantly different between the two groups. More proportions of volunteers tended to eat regularly and eat breakfast. Volunteers consumed seaweeds more frequently, and milk, fruits and fast foods less frequently than nonvolunteers. Our results indicate that some characteristics and dietary habits of volunteers are different from those of non-volunteers. However, we could not show that volunteers were more interested in their diet and had desirable dietary habits than non-volunteers. Further research on the characteristics of volunteers who participate in nutrition survey may be helpful to interpret and generalize the survey results.

Development of a Nutrition Education Website for Children

The purpose of this study was to develop a nutrition education website for children, especially those around the 4th-6th grade levels. Among the already existing websites providing nutritional information for children, 7 websites with comparable amounts of information were evaluated in terms of their topics, credibility, content, ease of use, and aesthetics. In addition, a survey was conducted to assess the need for nutrition information among 305 elementary students. The food pyramid was the topic offered most often on those websites. Information on nutrients, digestion, food safety, nutrition labeling, and healthy eating habits were available on the websites. Some of those websites also provided games or quizzes to attract the children's interest. However, seine of the websites did not give any information on the person responsible for providing the information, which is most important in determining the credibility of the website. Other problems were that some information was too difficult for children, and some of the websites did not provide a way to search for information. According to the survey results, the topics that children wanted to know about were 'desirable weight', 'cooking', 'correct food choice', 'weight control method', 'calories consumed during exercise' but, they did not have a high interest in 'asking questions', 'books with nutrition messages', 'under weight', 'negative effect of weight control' and 'helping hungry children'. Based on these results, we developed a new nutrition education web-site, ifood' (http://ifood.or.kr). We focused on helping children develop healthy eating habits by providing information which is easy and practical for children. However, we neither included the program to assess their dietary intake, nor provided entertaining background sounds, which children like. Also, the game section is composed mainly of quizzes. Therefore, we need to further develop exciting games to teach nutrition to children. In the near future, the newly developed website should be evaluated by children regarding understandability and coverage of the contents as well as usability and design, and then revised to improve the educational effect.

The Economic Benefits of Breastfeeding

To compare the costs incurred by infant feeding between mothers who breastfed their infants and those who fed them infant formula, a questionnaire survey was carried out to 136 mothers living in Seoul, Cheongju and Chungju who breastfed and 199 mothers who formula-fed their infants. The cost of formula-feeding was estimated based on the expenditures for formula and feeding apparatus, and the time needed to wash bottles and prepare formula. The cost of breastfeeding was estimated based on the expenditures for food for the additional nutritional intake of these mothers. The mean cost of formula-feeding was 1,870,125won during the first year of the baby's life. The food cost for the additional nutritional intake of the breastfeeding mothers was 203,004 won per year. The extra medical cost for respiratory illnesses in the formula-fed group compared to the breastfed group was 62,920 won because the formula-fed infants required medical attention for respiratory illnesses more often than the breast-fed infants. Therefore, breastfeeding could save 1,730,041 won during the first year of an infant's life. We may have underestimated the cost savings from breastfeeding because we did not take into account the potentially decreased costs of fertility control and the health benefits for mothers, as well as the decreased usage of water and gas. Analyses showed that breastfeeding is not only nutritionally advantageous, but also economically advantageous for families and society.

A Survey on the Feeding Practices of Women for the Development of a Breastfeeding Education Program: Breastfeeding Knowledge and Breastfeeding Rates

A survey was carried out to investigate the breastfeeding knowledge, breastfeeding rates and related factors among women in Seoul and the Chungbuk area, in order to obtain baseline data for the development of an education program on breastfeeding. Subjects included 671 lactating mothers who visited public health or pediatric centers, between December, 1999 and February, 2000 and were interviewed using a questionnaire. The results are as follows: In the breastfeeding knowledge, most of the subjects (93.4%) knew correctly that 'breastfeeding is better for a baby's emotional development than formula feeding,' whereas only half of the subjects (51.7%) answered correctly the question on whether 'formula is more nutritious than breastmilk.' The breastfeeding rates were about 57%, 40%, 33% and 12%, respectively for baby's aged 1, 3, 6, and 12 months. These results showed that the breastfeeding rate rapidly decreased with the baby's age. Lower breastfeeding rates were associated with high educational level (college graduate and above), high income, mothers' employment, no experience of attending breastfeeding education programs, and no planning on breastfeeding prior to pregnancy. There was a significant relationship between the level of knowledge and breastfeeding practice; the higher the knowledge score, the higher the breastfeeding rate. In conclusion, an education program should be developed and offered for each low breastfeeding rate group, in order to promote breastfeeding.

Content Analysis of the Questionnaires Used in Dietary Surveys

The purpose of this study is to analyze the contents of the questionnaires used in dietary surveys and to evaluate each item in relation to the item construction strategy. Articles of which the contents were related to food, nutrition, diet, dietary behavior, and related areas. Published from 1997 to 1999 were searched fir and a total of 121 questionnaires were collected and analyzed. The questions in the questionnaires were classified into related areas and sub-areas. Among the keywords in the title of the articles, the term 'nutritional status'(or 'dietary intake status') was most frequently used. The terms such as dietary status, obesity, health, food habit, and dietary behavior were also frequency used. Major topics of the items in the questionnaires varied according to the life cycle of the subjects of the study. The topics most frequently asked in each lift cycle were as follows : overeating, snack, and food preference for preschool- and school-aged children ; anthropometry, weight control, and snack for middle and highschool students : meal skipping, smoking, and drinking for college students : disease, smoking, drinking, and exercise for adults : and smoking, drinking, disease and perceived health for the elderly. Inappropriate questions with complicated language, typographic and grammatic errors, unnecessary words, and negative questions were found. Therefore, care should be taken to construct each question so as to avoid possible misinterpretation. Also, a standardized questionnaire be developed for survey researchers.

Development of a Simple Screening Test for Identifying Korean Elderly at Risk of Undernutrition

The develop a simple screening test for identifying Korean elderly at risk of undernutrition, the data of the health-related habits, dietary behaviors and nutrient intakes of the elderly in Chongju were analyzed. Two risk indicators, mean adequacy ratio (MAR) and the respondents perceived health, were used to detect the undernutrition risk of the elderly. In order to select a list of questions for the test, factors a affecting nutritional status and items investigated in previous nutrition surveys of the elderly were considered, and 21 questions were primarily selected. A multiple regression and stepwise regression analysis were used to take out the weak predictors of poor nutrient intake, and to give item weights to the strong predictors, and a list of 17 questions was finally adopted. To determine the cut-point of the test score, sensitivity, and positive predictive values were calculated. The Simple Screening Test developed in this study is a brief, easily scored tool to predict poor nutrient intake and the perceived health status of the elderly. The test may provide a basis of further comprehensive nutritional assessment or intervention planning, if necessary, for those who are diagnosed ad "high risk". The test, however, needs to be independently validated by other groups of individuals.

Blood Folate Level Determined by a Microplate Reader and Folate Intake Measured by a Weighted Food Record

Microbiological method using a 96-well microplate reader for folate assay was established, and folate intake and blood folate concentrations of 23 female college students were assessed. To evaluate folate intake, dietary data were collected by a 3-day weight food record, and serum and RBC folate concentrations were measured by the new method. The coefficient of variation for the new method was less than 10%. Mean daily folate intake of the subjects was 126.7ug which is only 50.7% of the RDA. The mean concentrations of serum and RBC folate were 7.46ng/ml and 294.4ng/ml, respectively, which were within the normal range. These results indicate that folate intake seems to be underestimated due to incomplete food composition database. Therefore, folate database should be appropriately in order to asses folate intake accurately.
