Accuracy of 24-hour Diet Recalls for Estimating Energy Intake in Elderly Men using the Doubly Labeled Water Method

OBJECTIVES
This study assessed the accuracy of the 24-hour diet recall method for estimating the energy intake of elderly men using the doubly labeled water as a reference method.
METHODS
Seventeen subjects (mean age 72.5 ± 3.9 years), who maintained the same body weight during the two weeks study period, were included in this study. Three 24-hour diet recalls (two weekdays and one weekend) were obtained over a 14 day period to estimate the mean energy intake. The total energy expenditure was measured over the same 14 days using the doubly labeled water method. The total energy intake and total energy expenditure were compared by paired t-test.
RESULTS
The total energy intake from the 24-hour diet recalls method was 2536.7 ± 350.6 kcal/day, and the total energy expenditure from the doubly labeled water method was 2659.8 ± 306.8 kcal/day. The total energy intake was slightly under-reported by −123.2 ± 260.8 kcal/day (−4.4%). On the other hand, no significant difference was observed between the total energy intake and total energy expenditure of the subjects (p=0.069). The percentage of accurate predictions was 64.7%. The correlation between the total energy intake and total energy expenditure was statistically significant (r=0.697, p < 0.005).
CONCLUSIONS
The present study supports the use of the 24-hour diet recall method to estimate the mean energy intake in elderly men group. More studies are needed to assess the validity of 24-hour diet recall method in other population groups, including elderly women, adults and children.

Comparison of Physical Activity Level, Physical Activity Pattern and Energy Expenditure in Male and Female Elementary School Soccer Players using Accelerometer and Physical Activity Diary

OBJECTIVES
The purpose of this study was to compare the physical activity level (PAL), the physical activity pattern and the energy expenditure in male and female elementary school soccer players using the accelerometer and the physical activity diary.
METHODS
Twenty-five (male 11, female 14) elementary school soccer players (9–12 years) participated in this study. During their daily activities, they wore an accelerometer for seven days (five weekdays and two weekend days) and completed the physical activity diary for three days (two weekdays and one weekend day). PAL was calculated by using the physical activity diary and three equations (Pate Preschool, Freedson Children, and Freedson) were used to calculate the intensity of physical activity and energy expenditure from the counts of accelerometer.
RESULTS
The average of physical activity by day of the week, CPM (Counts Per Minute) and METs (Metabolic Equivalents) were significantly higher in males (723.3 ± 149.2 CPM, 2.07 ± 0.18 METs) compared to females (505.6 ± 119.9 CPM, 1.79 ± 0.20 METs), but there was no significant difference in PAEE (Physical Activity Energy Expenditure) between the two groups (males: 15.5 ± 9.1 kcal/day, females: 11.5 ± 6.0 kcal/day). During weekdays, physical activity intensity was significantly higher in males compared to females at lunch time and training time than at other times. In both genders, the PAL was higher during weekdays (males 1.98, females 1.89) compared to weekend (males 1.62, females 1.61) (p<0.05).
CONCLUSIONS
Our study observations suggest the necessity to develop an accelerometer equation for accurately evaluating the physical activity of elementary school athletes.

Accuracy of Accelerometer for the Prediction of Energy Expenditure and Activity Intensity in Athletic Elementary School Children During Selected Activities

OBJECTIVES
Accurate assessment of energy expenditure is important for estimation of energy requirements in athletic children. The objective of this study was to evaluate the accuracy of accelerometer for prediction of selected activities' energy expenditure and intensity in athletic elementary school children.
METHODS
The present study involved 31 soccer players (16 males and 15 females) from an elementary school (9-12 years). During the measurements, children performed eight selected activities while simultaneously wearing the accelerometer and carrying the portable indirect calorimeter. Five equations (Freedson/Trost, Treuth, Pate, Puyau, Mattocks) were assessed for the prediction of energy expenditure from accelerometer counts, while Evenson equation was added for prediction of activity intensity, making msix equations in total. The accuracy of accelerometer for energy prediction was assessed by comparing measured and predicted values, using the paired t-test. The intensity classification accuracy was evaluated with kappa statistics and ROC-Curve.
RESULTS
For activities of lying down, television viewing and reading, Freedson/Trost, Treuth were accurate in predicting energy expenditure. Regarding Pate, it was accurate for vacuuming and slow treadmill walking energy prediction. Mattocks was accurate in treadmill running activities. Concerning activity intensity classification accuracy, Pate (kappa=0.72) had the best performance across the four intensities (sedentary, light, moderate, vigorous). In case of the sedentary activities, all equations had a good prediction accuracy, while with light activities and Vigorous activities, Pate had an excellent accuracy (ROC-AUC=0.91, 0.94). For Moderate activities, all equations showed a poor performance.
CONCLUSIONS
In conclusion, none of the assessed equations was accurate in predicting energy expenditure across all assessed activities in athletic children. For activity intensity classification, Pate had the best prediction accuracy.

Measurement of Energy Expenditure Through Treadmill-based Walking and Self-selected Hallway Walking of College Students - Using Indirect Calorimeter and Accelerometer

OBJECTIVES
The objective of this study was to assess energy expenditure and metabolic cost (METs) of walking activities of college students and to compare treadmill based walking with self-selected hallway walking.
METHODS
Thirty subjects (mean age 23.4 ± 1.6 years) completed eight walking activities. Five treadmill walking activities (TW2.4, TW3.2, TW4.0, TW4.8, TW5.6) were followed by three self-selected hallway walking activities, namely, walk as if you were walking and talking with a friend: HWL (leisurely), walk as if you were hurrying across the street at a cross-walk: HWB (brisk) and walk as fast as you can but do not run: HWF (fast) were performed by each subject. Energy expenditure was measured using a portable metabolic system and accelerometers.
RESULTS
Except for HWF (fast) activity, energy expenditures of all other walking activities measured were higher in male than in female subjects. The lowest energy expenditure and METs were observed in TW2.4 (3.65 ± 0.84 kcal/min and 2.88 ± 0.26 METs in male), HWL (leisurely) (2.85 ± 0.70 kcal/min and 3.20 ± 0.57 METs in female), and the highest rates were observed in HWF (fast) (7.72 ± 2.81 kcal/min, 5.84 ± 1.84 METs in male, 6.65 ± 1.57 kcal/min, 7.13 ± 0.68 METs in female). Regarding the comparison of treadmill-based walking activities and self-selected walking, the energy expenditure of HWL (leisurely) was not significantly different from that of TW2.4. In case of male, no significant difference was observed between energy costs of HWB (brisk), HWF (fast) and TW5.6 activities, whereas in female, energy expenditures during HWB (brisk) and HWF (fast) were significantly different from that of TW5.6.
CONCLUSIONS
In this study, we observed that energy expenditure from self-selected walking activities of college students was comparable with treadmill-based activities at specific speeds. Our results suggested that a practicing leisurely or brisk walking for a minimum of 150 minutes per week by both male and female college students enable them to meet recommendations from the Physical activity guide for Koreans.

Thermic Effect of Food, Macronutrient Oxidation Rate and Satiety of Medium-chain Triglyceride

OBJECTIVES
The objective of this study was to evaluate the thermic effects, the macronutrient oxidation rates and the satiety of medium-chain triglycerides (MCT).
METHODS
The thermic effects of two meals containing MCT or long-chain triglycerides (LCT) were compared in ten healthy men (mean age 24.4 +/- 2.9 years). Energy content of the meal was 30% of resting metabolic rate of each subject. Metabolic rate and macronutrient oxidation rate were measured before the meals and for 6 hours after the meals by indirect calorimetry. Satiety was estimated by using visual analogue scales (VAS) at 8 times (before the meal and for 6 hours after meal).
RESULTS
Total thermic effect of MCT meal (42.8 kcal, 8.0% of energy intake) was significantly higher than that (26.8 kcal, 5.1% of energy intake) of the LCT meal. Mean postprandial oxygen consumption was also significantly different between the two types of meals (MCT meal: 0.29 +/- 0.35 L/min, LCT meal: 0.28 +/- 0.27 L/min). There were no significant differences in total postprandial carbohydrate and fat oxidation rates between the two meals. However, from 30 to 120 minutes after consumption of meals, the fat oxidation rate of MCT meal was significantly higher than that of the LCT meal. Comparison of satiety values (hunger, fullness and appetite) between the two meals showed that MCT meal maintained satiety for a longer time than the LCT meal.
CONCLUSIONS
This study showed the possibility that long-term substitution of MCT for LCT would produce weight loss if energy intake remained constant.

The Measurements of the Resting Metabolic Rate (RMR) and the Accuracy of RMR Predictive Equations for Korean Farmers

OBJECTIVES
The purpose of this study was to measure the resting metabolic rate (RMR) and to assess the accuracy of RMR predictive equations for Korean farmers.
METHODS
Subjects were 161 healthy Korean farmers (50 males, 111 females) in Gangwon-area. The RMR was measured by indirect calorimetry for 20 minutes following a 12-hour overnight fasting. Selected predictive equations were Harris-Benedict, Mifflin, Liu, KDRI, Cunningham (1980, 1991), Owen-W, F, FAO/WHO/UNU-W, WH, Schofield-W, WH, Henry-W, WH. The accuracy of the equations was evaluated on the basis of bias, RMSPE, accurate prediction and Bland-Altman plot. Further, new RMR predictive equations for the subjects were developed by multiple regression analysis using the variables highly related to RMR.
RESULTS
The mean of the measured RMR was 1703 kcal/day in males and 1343 kcal/day in females. The Cunningham (1980) equation was the closest to measured RMR than others in males and in females (males Bias -0.47%, RMSPE 110 kcal/day, accurate prediction 80%, females Bias 1.4%, RMSPE 63 kcal/day, accurate prediction 81%). Body weight, BMI, circumferences of waist and hip, fat mass and FFM were significantly correlated with measured RMR. Thus, derived prediction equation as follow: males RMR = 447.5 + 17.4.Wt, females RMR = 684.5 - 3.5.Ht + 11.8.Wt + 12.4.FFM.
CONCLUSIONS
This study showed that Cunningham (1980) equation was the most accurate to predict RMR of the subjects. Thus, Cunningham (1980) equation could be used to predict RMR of Korean farmers studied in this study. Future studies including larger subjects should be carried out to develop RMR predictive equations for Korean farmers.

Relationships among Serum Adiponectin, Leptin and Vitamin D Concentrations and the Metabolic Syndrome in Farmers

The purpose of this study was to investigate the relationships among serum adiponectin, leptin and vitamin D concentrations and the metabolic syndrome in Korean farmers. 105 (26 males, 79 females) farmers (39~78 years, mean age 59.4 +/- 9.6 years) in Gangwon - area were included in this study. Anthropometric measurements and biochemical blood analysis of subjects were carried out. The prevalence of obesity, abdominal obesity, hypertension, diabetes, hypertriglyceridemia, hypercholesterolemia and hyper LDL-cholesterolemia, metabolic syndrome were 51.9%, 65.7%, 49.5%, 15.3%, 17.3%, 13.5%, 11.5% and 40.9%, respectively. Serum adiponectin and leptin levels (8.90 microg/ml and 12.6 ng/ml) of females were significantly higher than those (6.49 microg/ml and 4.88 ng/ml) of males. But there was no significant difference in 25(OH)vitamin D concentration between males (15.4 ng/ml) and females (16.9 ng/ml). In the subjects with metabolic syndrome, the adiponectin levels were significantly lower and leptin levels were significantly higher than those of the subjects without metabolic syndrome. Serum adiponectin level had positive correlations with HDL-cholesterol level (r = 0.325, p < 0.001), but showed negative correlations with triglyceride and fasting blood glucose concentrations, body weight and waist/hip circumference ratio (r = -0.202 ~ -0.317, p < 0.05). Serum leptin and 25(OH)vitamin D concentrations were positively correlated with body fat (kg, %) and BMI, waist and hip circumferences (r = 0.244 ~ 0.682, p < 0.001). The results of this study suggested that adiponectin and leptin levels could be credible indices to predict chronic diseases in farmers. However, further research on vitamin D should be carried out considering another factors.

Correlation of Nutrient Intake, Obesity-related Anthropometrics, and Blood Lipid Status with Instant Coffee-mix Intakes in Gangneung and Samcheok Residents

This study was conducted to investigate instant coffee-mix intakes and its relations with anthropometric measurements, nutrient intake, and blood lipid in Gangneung and Samcheok residents. Coffee-mix is a mixture of instant coffee, sugar and coffee-creamer. It is a standardized convenience food in Korea. Recently, Koreans appear to consume too much of instant coffee-mix. The respondents of this study were 218 Koreans (Gangneung region 133, Samcheok region 85). The average age was 56.0 years (male), 57.5 years (female). They were divided into 2 groups, the instant coffee-mix drinkers and the non-drinkers. The nutrient intake and anthropometric measurements and their relations with coffee-mix intakes were analyzed (nutrient intakes were calculated by converting per 1,000 kcal and adjusted by age, region). As for male, body fat was significantly higher in coffee-mix drinkers (18.1 kg, 25.0%) than in nondrinkers (17.1 kg, 23.8%). In the case of females, serum triglyceride was significantly higher in coffee-mix drinkers (109.1 mg/dL) than in nondrinkers (102.9 mg/dL). Coffee-mix intakes and coffee-mix's contributions to energy were higher for participants from Samcheok (21.2 g, 4.7%) than those from Gangneung (16.6 g, 3.4%). In addition, for Samcheok, coffee-mix's contribution to energy was significantly higher in male (5.6%) than in female (4.2%). Coffee-mix intakes and energy contribution of coffee-mix were not significantly correlated with anthropometric measurements. Only HDL-cholestrol showed significantly negative correlation with coffee-mix's contributions to energy. Based on the above results, we conclude that usual instant coffee-mix intake may increase intakes of sugar and fat and exert negative effects on serum lipids. Therefore, it is needed to control too much instant coffee-mix intake in the dietary management of rural residents.

Assessment of Energy Intake and Physical Activity Level for Korean Farmers to Establish Estimated Energy Requirements during the Off-Season for Farmers

The purpose of this study was to assess the physical activity level of Korean farmers to establish estimated energy requirements during the off-season. Subjects were 90 healthy males (n = 25) and females (n = 65). Body weight, height and body fat and muscles of subjects were measured. The prevalence of obesity among farmers was 56.7% according to the BMI. The farmers spent about 18 hours 7 minutes (75.5%) in sleeping and resting. The farmers spent about 19 hours 56 minutes (83.1%) out of 24 hours (one day) in "sedentary activities" and spent about 3 hours 56 minutes (16.4%) in "light activities". Physical activity level (PAL, activity coefficient) of female farmers was 1.42 which was not significantly higher than that (1.37) of male farmers. Estimated energy requirements (EER) for farmers who were different in age groups and gender were calculated. For example, the EERs for male and female farmers were 2190 kcal/day and 1712 kcal/day, respectively. The daily energy intakes of male and female farmers were 1803 kcal/day and 1610 kcal/day, respectively. The EER of male farmers was 2190 kcal/day which was significantly higher than that (1803 kcal/day) of the recommended daily energy intake of male farmers. The results of this study suggest that estimated EER of farmers should be modified according to seasonal workload and energy balance of farmers should be evaluated to prevent obesity.

The Relationship among Insulin Resistance, Blood Profiles and Nutrient Intake in Overweight or Obese Children and Adolescents

The purposes of this study were to investigate blood profiles and nutrient intakes of groups that are different in obese levels, and to find the credible predictor of insulin resistance. The subjects were classified as normal weight (%IBW < or = 110), obese without MS and obese with MS according to IDF definition of the risk group in metabolic syndrome (MS). Subjects of this study were included 137 (59 boys, 78 girls) free living children and adolescents (mean age 12.6 +/- 3.4 years) in Gangneung area, South Korea. %IBW of normal weight (94.9%), obese without MS (123.8%) and obese with MS (131.5%) were significantly different among groups. HOMA-IR had positive correlations with TG (r = 0.634), waist circumference (r = 0.553), atherogenic index (r = 0.513), %IBW (r = 0.453) and ALT (r = 0.360), but showed negative correlations with HDL cholesterol (r = -0.417, p < 0.001). HOMA-IR showed positive correlation with polyunsaturated fatty acid intake (p < 0.05). The energy intake of obese with MS was 1762 kcal/day which was not significantly different from those of normal weight and obese without MS. Total fatty acid intakes of two obese groups were significantly higher than that of normal weight. The results of this study suggest that waist circumference and ALT as well as TG, atherogenic index and weight can be credible indices to predict the insulin resistance in children and in adolescents. In addition, nutrition education and adequate diet should be provided to prevent MS in children and in adolescents.

Thermic Effect of Food, Macronutrient Oxidation Rate and Satiety of High-fat Meals with Butter and Sesame Oil on Healthy Adults

The purpose of this study was to measure and investigate the acute effects of two fatty meals (high-SFA & high-PUFA) on post-prandial thermic effect, substrate oxidation, and satiety. Eight healthy adults (four males and four females) aged 19-22 years were assigned to consume two isocaloric meals: high in saturated fatty acids from butter and high in polyunsaturated fatty acids from sesame oil. Indirect calorimetry was used to measure resting energy expenditure (REE), post-prandial energy expenditure for five hours, and substrate oxidation. Satiety of the subjects after meals was estimated by using visual analogue scales (VAS). Five hours thermic effect of food (TEF) was not significantly different between butter meal (6.5% of energy intake) and sesame oil meal (7.3% of energy intake), but, the TEF of butter meal reached the peak point at 150 min and decreased more rapidly arriving to REE in 270 min. On the other hand, TEF of sesame oil meal reached the peak at 90 min and decreased slower than butter meal (still higher than REE at 300 min). No significant differences in substrate oxidation rates were found between the two meals. Post-prandial fat oxidation rates increased significantly after the consumption of both butter and sesame oil meal than that of the pre-prandial state. Satiety values (hunger, fullness, and appetite) were similar among the meals, but recovery of hunger and fullness to the pre-prandial state was faster in butter meal than that of the sesame oil meal.

The purpose of this study was to assess the physical activity level of Korean farmers to establish their estimated energy requirements during busy farming season. 113 farmers (mean age 51.9 +/- 7.2 years, male 42, female 71) who own farmland area above 300 pyung participated in measurement of body weight and height and interviewing of one-day activity diaries. There was no significant difference in age, BMI between male and female. Obesity prevalence according to BMI among male and female farmers was 40.5% and 50.7% respectively. The farmers spent about 7 hours in sleeping and spent about 4 hours 35 minutes working on the farm and spent about 15 hours 30 minutes (64.6% of 24 hours) in "very light activities" and spent about 3 hours 31 minutes (14.7%) in "light activities". Physical activity level (PAL, activity coefficient) of male farmers was 2.63 which was significantly higher than that (2.19) of female farmers (p < 0.05). Estimated energy requirements (EER) for farmers who were different in age and gender were suggested. For example, the estimated energy requirements for male and female farmers were 3058 kcal/day and 2279 kcal/day, respectively. The results of this study suggest that estimated energy requirements (EER) of farmers should be differentiated according to seasonal workload and energy balance of farmers should be evaluated to prevent obesity.

Assessment of Physical Activity, Activity Coefficient of Preschool Children and Actual Condition of Daycare Center Outdoor Play

The purpose of this study was to assess physical activities and activity coefficient of preschool children and was to give some concrete information to activate outdoor play and to probe the suggestions to activate outdoor play. 42 preschool children (17 boys and 25 girls) were included. Body weight and height were measured and one-day activity diaries were collected by interviewing with their parents and kindergarten teachers. To measure physical activity during outdoor play, this study was researched the current conditions and content of outdoor play activity in daycare centers, and teacher's perceptions of that outdoor play. The average age of the subjects was 5.0 years. Average height, weight, body fat and body muscle of subjects were 108.9 cm, 18.9 kg, 17.7% and 26.8%, respectively. The subjects spent about 10 hours 39 minutes sleeping; 4 hours 16 minutes personal hygiene and computer working. They spent 88.2% of 24 hours (one day) in "very light activities" and 11.3% in "light activities". Activity coefficient (1.34) of weekday was significantly higher than that of weekend (1.21). And in this study, it was found that most of daycare centers (87.7%) have outdoor play space, 95.2% of them were garden. The teachers recognized that have a outdoor play frequency everyday (48%), three or four times (46%) in a week but they have three or four times (38%), one or two times (30%) because of various indoor program. Also, they recognized that have a outdoor play expending time 21- 30 minutes (48%), 31-40 minutes (26%) but they have 21-30 minutes (64%). They answered that have frequency and expending time less than their recognition of outdoor play. Most of daycare center teachers perceived outdoor play to be as important as indoor play, and the teachers believed outdoor play supports children's physical, cognitive, social, and language development. The results of this study may be used to utilize as a basic data for estimate physical activity for preschool children and developed that exercise program to increase physical activity of daycare center outdoor play.

Accuracy of Predictive Equations for Resting Metabolic Rate in Korean College Students

The purpose of this study is to analyze the accuracy of predictive equations for resting metabolic rate (RMR) in Korean college students. Subjects were 60 healthy Korean college students (30 males, 30 females) aged 18-25 years. RMR was measured by indirect calorimetry. Predicted RMRs were calculated using the Harris-Benedict, Schofield (W)/(WH), FAO/ WHO/UNU(W)/(WH), Owen, Mifflin, Cunningham, Liu, IMNA and Henry (W)/(WH) equations. The accuracy of the equations was evaluated on basis of accurate prediction (the percentage of subjects whose RMR was predicted within90% to 110% of the RMR measured), mean difference, RMSPE, mean % difference, limits of agreement of Bland- Altman method between predicted and measured RMR. Measured RMR of male and female students were 1833.4 +/- 307.4 kcal/day and 1454.3 +/- 208.0 kcal/day, respectively. All predictive equations underestimated measured RMR. Of the predictive equations tested, the Harris-Benedict equation (mean difference: -80.4 kcal/day, RMSPE: 236 kcal/day, mean % difference: -3.1%) was the most accurate and precise, but accurate prediction of the equation was only 42%. Thus, this study suggests that the ethnicity-specific predictive equation from Korean people should be developed to improve the accuracy of predicted RMR for Koreans.

A Study on Dietary Attitudes and Dietary Self-Efficacy of Juvenile Delinquents

The purpose of this study was to identify the factors associated with dietary attitudes and dietary self-efficacy of juvenile delinquents. Out of 110 questionnaires distributed to the juvenile detention home students in Daegu, and 150 questionnaires distributed to the juvenile detention home students in Anyang, 210 were analyzed (80.7% analysis rate). These data were analyzed by SPSS program. The results are summarized as follows. Almost all parents' educational levels showed 'graduation of middle school', and only 43.8% of the juvenile detention students were living with parents. In case of incoming status, average income per month was 2,000,000 won, which was lower than normal one. Diet attitude score was higher in boy students than girl students (p < 0.05). There was a higher score of diet attitude in higher educational levels of father. The average score of dietary self-efficacy was 2.51 +/- 0.86. This value was lower than the normal juvenile. There was a higher diet attitude score in the higher dietary self-efficacy score (p < 0.001). Therefore, proper nutrition education is required to improve their dietary self-efficacy. The results of this study could be useful data to plan and develop nutritional education programs of juvenile delinquents

Assessment of Daily Steps, Physical Activity and Activity Coefficient of the Elementary School Children in the Rural Area

The purpose of this study was to assess the daily steps, physical activities and activity coefficient of the elementary school children in the rural area. Body weight, height and daily steps were measured and one-day activity diaries were collected by interviewing children. The average age of the subjects was 9.96+/-1.02 years. Average height, weight, obesity index, body fat and muscle of subjects were 136.2+/-8.9 cm, 35.2+/-8.5 kg, 99.6+/-18.8%, 22.9+/-8.5% and 35.8+/-6.0%, respectively. The average daily steps of the subjects was 17,584 and daily steps (19,314) of 3rd grade students was significantly higher than that (15,712) of 5th grade children. But there was no significant difference in daily steps and activity coefficients between boys and girls. Daily steps (23,347) of exercise group showed the significantly higher than that (16,144) of nonexercise group. Gender and grade of subjects did not have significant influence on activity coefficients, but there was a significant difference in activity coefficient on weekdays between the exercise group (1.82+/-0.30) and non-exercise group (1.50+/-0.21). Analysis of variance revealed stronger associations between daily steps and body fat (%) than between daily steps and BMI. Daily steps showed significant negative correlation with body fat (%) measured using two methods r =-0.321 and r = -0.365, respectively. Activity coefficient was significantly correlated (r = 0.436) with daily steps, thus increasing daily steps can prevent and treat childhood obesity by increasing the energy expenditure. The higher activity coefficients (weekday 1.56, weekend 1.53) of the subjects was caused by the fact that rural students spent more time outside and enjoyed stronger activities than urban students. The results of this study can be used to estimate energy requirements for Korean children based on exercise levels and living areas.

Purpose
of this study was to compare nutrient intakes of disabled children and non-disabled children. Subjects consisted of 86 disabled children from a special education school and 127 non-disabled children from an elementary school in Seoul. Nutrient intakes were assessed by modified 24-hr recall method, with the help of children's parents and teachers. Almost all nutrient intakes (energy, protein, fat, carbohydrates, vitamin B1 and niacin) of children with cerebral palsy were significantly lower than those of other groups. But nutrient intakes per body weight of children with cerebral palsy were not significantly different with those of other groups. There was no significant difference between disabled and non-disabled children in almost % RDA (rate of actual intake to RDA) except of energy %RDA in children with cerebral palsy. NARs (nutrient adequacy ratio) for energy and vitamin B1 of children with cerebral palsy were significantly lower than those of children with autism and mental retardation, and non-disabled children. The proportions of energy, carbohydrate and protein intakes from lunch were significantly higher than those from breakfast and dinner in children with mental retardation and autism. The nutrient intakes of disabled children were different between other groups according to the type of handicap. For example, children with cerebral palsy had the risk of undernutrition. On the other hand, autistic children had the tendency of overnutrition. These results suggest that nutrition educational programs and educational materials for disabled children, their teachers and their parents should be developed considering the type of handicap.

A Comparison of the Eating Habits and Eating Behaviors of Disabled and Non-disabled Children

This study was conducted to compare the eating habits of disabled and non-disabled children in Seoul and Gangneung, Korea. Questionnaires about eating habits were answered by the children's parents and their teachers. The subjects of this study consisted of 146 disabled children (108 boys and 38 girls) from two special education schools and 241 nondisabled children (control group, 120 boys and 121 girls) from two elementary schools in Seoul and Gangneung, respectively. The percentage of the children who required more than 30 minutes to eat was 11.3% in the disabled group and 2.5% in the non-disabled group. In the disabled group, 44.0% ate excessive amounts of food or could not control their intake. The percentage of the children whose frequency of eating breakfast was less than 1 to 2 times per week was 21.0% in the disabled group and 9.7% in the non-disabled group. Also, 7.6% of the disabled group and 13.9% of the nondisabled group had snacks more than three times per day. The percentage of children who were able to eat by themselves was lower in the disabled group (47.9%) than in the non-disabled group (87.8%). Of the remainder of the disabled group, 28.6% spilled food, and 14.3% needed the aid of others when picking up side dishes. The percentage of parents who worried about their children's eating an unbalanced diet was 48.5% in the disabled group and 41.8% in the non-disabled group. In addition, there were problems with eating behaviors in 22.7% in the disabled group ; and with under-eating (15.9%) and with excessive intake of instant foods (16.8%) in the non-disabled group. These results suggest that the eating habits and eating behaviors of disabled children are different from those of non-disabled children. Thus, nutritional educational programs and educational materials for disabled children and their parents should be developed.

Comparison of Anthropometry, Serum Lipid Levels and Nutrient Intakes of Two Groups Based on their Drinking, Smoking, Exercise, Menopause and Obesity Status: In Residents of Youngdong Area

The purpose of this study was to compare the anthropometry, the serum lipid levels and the nutrient intakes of two groups according to their alcohol intake, smoking habits, regular exercise patterns, menopause status and obesity levels. The subjects consisted of 180 clinically normal adults (males 89, females 91) in the Youngdong area. There were significant differences in the triglyceride (TG) concentrations between the alcohol consumers (169.3 +/- 130.0 mg/dl) and the non-alcohol consumers (111.4 +/- 64.5 mg/dl), and the smokers (165.0 +/- 103.6 mg/dl) and the non-smokers (110.8 +/- 39.0 mg/dl). Although regular exercise did not affect the serum lipid levels, the dietary intakes of cholesterol, niacin and phosphate were significantly higher (p < 0.05) in the male exercising group than in the male non-exercising group ; and female exercising group consumed more polyunsaturated fatty acids (p < 0.05) than the female non-exercising group. The waist/hip ratio, serum total cholesterol (198.4 +/- 36.3 mg/dl) and serum LDL-cholesterol (119.5 +/- 34.9 mg/dl) levels of the postmenopausal females were significantly higher than those of the premenopausal females (p < 0.01). The obese males showed higher serum cholesterol, LDL-cholesterol, LDL/HDL, and relative cholesterol levels than those of the normal-weight males ; and the obese females showed higher serum TG levels than that of the normal-weight females (p < 0.01). These results suggested that drinking, smoking, menopause and obesity are risk factors for hyperlipidemia. Thus, moderation in alcohol consumption, non-smoking, regular exercise and the maintenance of normal weight are necessary to prevent hyperlipidemia in middle-aged people.

A Study on the Obesity and Stress of Elementary School Children in the Kangnung Area

This study was carried to estimate the prevalence of obesity and to investigate the relationship between stress and obesity in children. The subjects were 508 children, who were randomly selected from the fifth and sixth grade at nine elementary schools in Kangnung. The height, body weight, waist and hip circumferences and body fat(%) were measured and the levels of stress were assessed by a questionnaire consisting of the following 3 domains ; 4 items for personal factors, 11 for home, and 18 for school. The prevalence of obesity in male and female children greatly varied by the indices from 2.5% and 1.7% when judged by BMI, 9.5% and 4.0% by obesity index, and 29.7% and 34.4% by body fat(%). The stress scores from personal(47.1%) and school (47.9%) factors were higher than the stress score from home(38.5%). The stress score from personal factors of female children is significantly higher than that of male children, but the stress score from home of female children is significantly lower than that of male children. The personal stress score and the total stress score of obese female children were significantly higher than those of non-obese female children. In male children, the obesity index(%) or body fat(%) have a significant positive correlation with the stress scores from over-expectation of parents, a lack of conversation with family, examinations, discontent about school, teacher's partiality to students, and sexual curiosity. The female children have a significantly positive correlation of obesity index (%) or body fat(%) with stress scores from appearance, quarrels of parents, and inferiority complex to brothers or sisters. These results suggested that counselling and education about not only balanced diet but also the strategies for actively coping with stress are needed to prevent and treat childhood obesity.

Comparison of the Nutrition Knowledge, food Habits and Life Styles of Obese Children and Normal Children in Elementary School in Kyeong-gi Province

The nutrition knowledge and food attitude of obese children were assessed and compared with their normal children counterparts to provide information for nutrition education programs. One hundred forty six obese children and 92 normal children from the fourth to sixth grade in Bucheon and Anyang elementary schools of Kyeong-gi Province were selected for the study. A multiple choice questionnaire, including 12 items of general nutrition knowledge and 13 items of weight control, were used to test the nutrition knowledge of children. The results of this study are as follows : 1) 57.5% of male and 40.9% of female obese children had experience with weight control, but the rate of correct answers on the nutrition knowledge test ranged from 53% to 58%, which does not show any significant difference between the two groups. The rate of correct answers on items conrerning 'general nutrition knowledge' and 'nutrition knowledge for weight control' did not show any significant difference among the two groups. 2) Obese male students showed a higher rate of correct answers(59.2%) on items of 'the proper food selection for obese children', compared with their counterparts and also obese female students showed a higher rate of correct answers(88.6%) on 'physical exercise', compared with their counterparts. 3) The rate of correct answers on nutrition knowledge of 'basic food groups', 'the functions of iron and vitamins', 'the concept of energy', and 'the principle of weight change' were lower than 40%. 4) The obese children frequently did not have breakfast, overate during lunch in many cases, and had low recognition on knowledge that was necessary to weight control. 5) Only 30.9% and 22.7% of male and female obese children replied that they would participate in weight control programs. However, 53.6% of males and 68.2% of females did not show any concern in participating the programs. From these results, it was suggested that it is necessary to motivate the obese to participate in weight-control program. When developing nutrition education programs for the obese, the contents of the questions that showed a low correct answering rates should be emphasized.

A Study on Iron Nutritional Status of Girls at Puberty in Kangnung Area

This study was designed to assess the iron nutritional status of girls at puberty in Kangnung area. The subjects consisted of 161 adolescents in sixth-grade in primary school and first-grade in middle school girls. Anthropometric measurements were taken for body weight, height, percentage of body fat, and circumferences of waist and hip. Nutrient intakes were assessed by modified 24-hour recall method. Food models and other measuring tools were also used. Fasting blood samples were obtained and analyzed for hemoglobin(Hb) concentration, hematocrit(Hct), serum iron(FE) and total iron binding capacity(TIBC). Mean values for Hb, Hct, Fe, TIBC, TS and serum ferritin were 13.6+/-0.9g/dl, 39.6+/-3.9%, 91.3+/-36.3 microgram/dl, 327.9+/-45.2 microgram/dl, 28.3+/-11.8% and 37.4+/-24.2ng/ml, respectively. Prevalence of iron deficiency greatly varied by indices from 4.8% when judged by Hb to 18.4% by serum Fe concentration. The Hb concentration was positively correlated with Hct(r=0.641), serum iron(r=0.266) and transferrin saturation(r=0.237)(p<0.05). On the other hand, serum ferritin concentration showed significantly negative correlation with TIBC(r=-0.572). Mean daily intake of iron was 14.94mg and heme iron intake was 1.13mg and which was 7.6% of total iron intake. Total absorbable iron calculated by the method of Mosen was 1.38mg and bioavailability of diectary iron was 9.3%. These results suggest that the prevalence of iron deficiency of pubertal girls is very high, therefore the guidelines for diet and social supports, such as, school food service system should be provided to improve their iron status in middle school students.
